EMPLOYEE HAD LAST WORKED GRAVEYARD SHIFT 8-20-89. SHORTLY AFTER THE START OF HIS HIFT HIS SUPERVISOR HAD STOPPED IN TO CHECK ON HIM DURING THIS TIME, EMPLOYEE HAD STATED HE THOUGHT HE MIGHT HAVE APPENDICITIS HE DECIDED TO WORK ANYWAY. WE HAD NOT INJURY REPORT OR ANY INDICATION THIS WAS WRK INCURRED. ON THE 21ST HE HAD GONE TO DR ON THE 23RD HE HAD SURGERY AND IS OFF WORK NOW.

EMPLOYEE WAS SQUATTING DOWN IN AN ELEVATOR PIT WORKING. WHEN HE ATTEMPTED TO STAND UP, HIS RIGHT KNEE MAKE A LOUD SNAPPING SOUND AND LOCKED. HE HAS HAD PAIN SINCE.

EMP WAS WORKING3ON A STRONG-WINDY DAY, HIS EYES BECAME VERY IRRITATED DURING THE DAY. ALTHOUGH NO FOREIGN BODY WAS REMVOVED HE WAS REQUESTED TO BE OFF FOR 3 DAYS BECAUSE OF THE IRRITATION. HE WAS WEARING GOG1LES, BUT APPARENTLY NOT SUITED FOR THE WEATHER CONDITIONS THAT DAY. REPORTED AS A MEDICAL CASE BECAUSE TIME WAS LOST
