Showing all 5Employee passed out at work. Upon questioning & evaluation by shift foreman, employee attributed passing out to his diabetes. Employee refused medical treatment and went home at the end of his shift. From home he went to the doctors/hospital where it was determined he had a heart attack.
Employee went from the office out to line 2 mixer. He turned and his back went out. Employee states that he has bad disks in his back.
