Employee was turning on pump, when he was stung by a bee and due to being allergic, he was taken to the ER. Was not given completed accident form until 6/26/08.
Unknown - employee doesn't know of a specific incident that his pains began. He did say he was just walking and his knee began to hurt. In his opinion his problem is a result of shoveling the saw cuts.
EMPLOYEE WAS BLOCKING TRACK WHEN HE BECAME SHORT OF BREATH & EXPERIENCED STIFFNESS. HE TOLD THE SUPERVISOR HE DIDN'T FEEL WELL & LEFT. HE WENT TO THE E.R. TO GET CHECKED OUT & HAS TWO BLOCKED ARTERIES.
EE WAS WALKING APPROX. 100 FT TO BEGIN A NEW CUT WHEN HE COLLAPSED, AN EE STARTED CPR, AMBULANCE ARRIVED ABOUT 15 MIN AND TOOK TO HOSPITAL.
