Lime Plant maintenance employee got lime dust in eye. The weather was windy. EE was wearing safety glasses with side shields. EE flushed eye several times with no relief. Supervisor told EE to see a doctor. The doctor confirmed a chemical burn from the lime dust and gave employee prescription eye drops for treatment of any infection.

A windy day. The employee was walking to the parking lot at the end of the day to go home. He still had his PPE on. A gust of wind blew a speck of dirt into his eye. He went to the office and flushed his eye. He then drove home.

EE was conducting routine daily walk-around inspection of the primary crusher plant and conveyors. Supervisor came to check on EE and told him to take a break based on his appearance. EE then complained of chest and arm pains. 911 was called and EE was transported to hospital where he passed away later that night.
