Employee reportedly collapsed and was having trouble moving legs and arms on right side of body (symptoms associated with stroke). Transported to local ER and treated for dehydration/potassium deficiency. Employee working their first shift from days off, not considered work related and given full duty release to work. MSHA Hotline was notified just in case.

Employee had returned from taking samples, entered information, stood up from the chair and experienced a sharp pain run from lower left leg to lower hip area. Employee was evaluated by an ERT member then taken to the ER for evaluation. Employee was taken off work until evaluated by PCP.

While reaching down for a radio on the bottom shelf, employee felt a sudden pain in EE's lower back. Employee went home the remainder of the day, and then on days off. On 6/30, pain persisted, and employee went to the ER, was referred to PCP and taken off work. CURRENTLY ONGOING

Employee had just arrived at the office and told a co-worker was experiencing chest and arm pain. Was evaluated by ERRT and transported, met by ALS and flown to a cardiac hospital in Las Cruces NM. Courtesy notification to MSHA was made. Employee had surgery and should return next week. Personal - Code 8.

Employee was exiting service truck, as they stepped down they felt a pop in right knee. Employee was taken in to be checked, was released with restrictions and referred to ortho. EE was seen by Ortho 8/28/20 and given a full duty release.

9:00-9:05, blade operator requested supervisor or mine tech. When mine tech arrived, employee stated employee thought employee was having a heart attack. MayDay was called, taken to ER. 9:46 site notified no heartbeat, 10:55 declared deceased. Courtesy call to MSHA, inspector investigated for chargeability, stated likely personal illness at investigation close out.

Employee put glove on & felt something hit finger. Should have reported bite by end of shift however bug bites happen all the time & are not reported. It wasn't until days later that medical attention was needed.

Employee was checking back flow devices in the well field, when he opened the box on one BFD, bees came out and began stinging him. He called for help, was transported to the ER where over 70 stings were counted, and medical treatment administered. RTW regular shift 8/10/15.

Two employees were assigned to clean up a dry lime spill. One employee paused and removed his glasses, when a gust of went blew lime in his eye. The eye was flushed, and he was taken to the optometrist, and given a prescription.

Required PPE is a rubber boot for the area worked in. He was given a size 10 boot when he needed a size 10.5. He wore the boot instead of reporting the incorrect size. The boot rubbed a sore on the end of his left big toe that later became infected. He was given a daily antibiotic and a shot of antibiotics and released full duty.

Personal Illness. Employee was surveying when he became dizzy and he sat down on the berm. The employee was observed by a co-worker who went to the employee to check on him and he immediately called the supervisor to come to the area where the employee was located. A First Responder was contacted and a Mayday was called. The employee was transported to the area hospital.

Employee was bending over closing the drain on the vacuum system in the X-Ray sampling room and when he tried to stand back up, the employee said that he felt pain in his lower back and could not stand up straight.

Employee was washing on the feeder floor at the primary crusher. He turned around to wash a different spot and felt his ankle give and rolled it. He was taken to the doctor, diagnosed with strain, and given restricted duty until 08/15/08. (Same ankle employee injured in 2007)

At approx. 10:00, employee was driving haul truck to shop for lube service, and was not feeling well. He went to gen. supervisor and requested a 1st responder check his vitals. He then requested to be transported to ER at local hospital. On 4/7, supervisor was notified that he was going to be removed from life support. At that time, company notified MSHA.

EE was walking across the parking lot when she felt a sharp pain in her ankle. There was no slip trip or fall.She stated her ankle popped. She was transported to emergency room and diagnosed with a fracture. Currently ongoing.

While targeting a shot for a shovel, the wind blew a dirt particle into the ee right eye. EE was wearing proper PPE. It did not irritate her eye for the remainder of the shift. However, the ee was unable to remove the particle from her eye and it is now starting too be to painful. EE was taken to see the eye dr today. Nov 21, the particle was removed from the eye, Rx eye drops

EMPLOYEE WAS IN A KNEELING POSITION CALIBRATING A MARCY SCALE AT A DISTRIBUTION BOX AND FOUND IT DIFFICULT TO STAND UP. HE WAS TAKEN TO HIS PHYSICIAN, DIAGNOSED WIT A PRE-EXISTING CONDITION, WHICH WAS AGGRAVATED ATTEMPTING TO STAND BACK UP, HE WAS TAKEN OFF WORK FOR 5 DAYS, THEN RELEASED WITH A SEVEN DAY, LESS THAN 10 LBS LIFTING RESTRICTION

EE WAS CHANGING INTO HIS WORK CLOTHES, WHEN HE FELT SOMETHING BITE OR STING HIM ON THE RIGHT HAND. HE SHOOK HIS HAND BUT DID NOT SEE ANYTHING. EE WAS TAKEN INTO THE HOSPITAL AND GIVEN PRESCRI PTION LEVEL DOSAGE OF AN OVER THE COUNTER MEDICATION. EE WAS SCHEDULED FOR TRAINING THE FOLLOWING MORNING AND DID NOT COME IN COMPLAINING OF PAIN.
