Showing all 17Dust from inside the dust collector on top of the cement silo blew into the employees eyes. He was installing new bags at the time.
Ee is alergic to bee stings. Ee self administered an epipen injection and was transported to Hosptital where he had a full recovery before the doctor could see him. No doctor report filled. There is some question as to whether or not this should be reported. We will report it as an "other" even though it was not medically treated.
Employee was found @ 7:40am collapsed in tunnel walkway. Emergency procedures were admininstrated to no avail.Employee was pronounced dead at the site preliminary findings were by natural causes.Awaiting final determination.
Ee worked a double shift starting at 3Pm on 3/7 thru 6AM 3/8. By the end of the shift he was stooped over and walking strange. He reported it to the shift forman and told him he was "stiff from the cold night air". He went home and could not rest. Called the plant about noon and requested to be seen by a doctor. No specific incident remarks. Ee placed on light duty
EE FELT ILL & WANTED TO GO HOME. PAIN IN LEFT UPPER CHEST ALSO. EE LEFT WORKSITE AFTER TELLING FOREMAN. EE DID NOT GO HOME - SOUGHT OBSERVATION AT HOSPITAL. HOSPITAL NURSE NOTIFIED MINE OFFICE AT EE'S REQUEST AT 2:15 PM. MILD HEART WEAKNESS; PHYSICIAN: RETURN TO WORK OK AS OF 1/19/06.
Per call to K. Schreimer and Gordon Brown (plant manager): Actual date of accident unknown - occurred 6 mths ago. EE started restricted duty on 10/7/05. EE "wrenched" both shoulders but could not give specific date or incident. Condition became progressively worse over time. He will be having surgery for torn rotator cuffs in the future.
Breathing difficulties. EE provided first notification of this incident 7/26/05.
While walking across yard another employeee threw a football and it hit him in head knocking him down.
WHILE EMPLOYEE WAS TRYING TO HOOK A CABLE TO THE DREDGE, WAVES CAME UP AND TOOK THE SLACK OUT OF THE CABLE AND STRUCK EMPLOYEE.
NOTIFIED OFFICE ON 4/15/02 LOWER BACK & RIGHT LEG HAD A PAIN WHILE CLIMBING STAIRS TO MILL ON 4-19 HE AGGRAVATED IT AND REQUESTED TO SEE DR. AT THAT TIME.
