Showing all 14Injury was reported on 8/21/07 with no work restrictions. Aggravation of the injury, according to the doctor, caused the need for left hip surgery. Employee underwent left hip surgery on 4/27/09. (No accident or injury was reported after 8/21/07).

Employee was walking along 311 belt conveyor and miss stepped and twisted her right knee.

Employee is unsure of when alleged injury occurred. Company was informed on 5/5/06 of the alleged injury. Employee stated that he believes he pulled his back while walking out in the plant back on 5/2/06. He didn't experience any discomfort or pain until 5/5/06.
Employee was checking the operation of the stacker and when he bent over he felt a pain in his back.
EMPLOYEE WAS STANDING BESIDE MILL, PREPARING TO ASCEND LADDER WHEN CEMENT DUST FLEW INTO EYES.
EE WAS OPERATING THE GRADER AND WHEN THE DOOR OPENED THE WIND CAUGHT IT AND HE TRIED TO HOLD IT. THUS STRAINING HIS RIGHT SHOULDER.
EMPLOYEE WAS FOUND DEAD IN A BATHROOM AT TXI-RIVERSIDE CEMENT COMPANY. HE WAS FOUND WITH A SYRINGE STILL IN HIM. A BALLOON WITH REMNANTS THAT SEEM TO BE HEROIN, A SPOON, AND A RUBBER STRIP. AC CORDING TO THE DEPUTY CORONER AND THE POLICE, IT LOOKS LIKE A DRUG OVERDOSE. AN AUTOPSY WILL BE PERFORMED TO DETERMINE THE EXACT CAUSE OF DEATH.
