Employee was leaning forward to pass under a screw conveyor and felt a strain in his lower back. Conveyor is approximately 5 feet off the floor.

80 year old man reportedly, suffering from dementia, drove into the UG mine sometime between 3/20/08 and 3/24/2008. He became stuck and exited his van. Apparently he attempted to exit the mine with a flashlight. He apparently lost the batteries power, could not get out and was found dead the morning of 3/24/2008.

EE was walking up steps. When he got to the landing he turned to see if the belts were running and felt a popping and burning in his left knee. EE will have surgery for torn cartilage.

EE WAS IN THE PROCESS OF LOADING A PEBBLE TRUCK. AS HE WALKED FROM THE CONTROL PANEL BACK TO THE TRUCK THE WIND BLEW A PIECE OF LINE IN HIS EYE. EE'S RIGHT EYE WAS BURNED BY THE LIME. EE WAS W EARING SAFETY GLASSES AT THE TIME. WIND HAD BEEN GUSTING 10-15 MPH DURING DAY.
