IW stood up from a crouching position when he felt a pain in his lower leg. He was diagnosed with a tibia/fibula fracture.
Employee died of unknown (at this time) natural causes, not related to work.
An incident did not occur. EE complained of back pain at the end of shift.
Injured was preparing to jack up rear of rubber tired dozer. While positioning jack, his foot slipped on some dirt and he felt a pull in groin area. Result was swollen scrotum, testicle and bruising of groin area.
Employee was sitting in the driver's seat of a pickup when he reached behind him to grasp the seat belt and strained his shoulder. The injury was reported to the company on 11/5/2007. He was examined by the authorized provider on 11/5/2007. Physical therapy was prescribed and administered.
EMPLOYEE, A WELDER WAS WORKING ON WELDING A PIPE WHEN A GUST OF WIND PROPELLED SOME DEBRIS INTI HIS EYE. HE INTIALLY FELT SOME IRRITATION, BUT CONTINUED DOING HIS WORK.
