Employee stood upright after rolling a 12 inch belt and experienced a back spasm. Employee sought treatment at a chiropractor.

Employee was working in the backend building on the Z drag for Kiln 1 when a big gust of wind came through and blew dust debris in employee's left eye.

The employee was working on the vent dust collector and was going from hopper to hopper and was standing on metal grating. When EE turned to move EE's foot stayed where it was as EE continued to twist and EE heard a pop in EE's knee.

Operator trainee reported to supervisor EE was reaching overhead and felt a sharp pain in EE's left shoulder. Employee thought it might be a re-occurrence of an earlier injury sustained in Nov of 2019. Employee was referred to clinic, who recommended 2wks of physical therapy. Employee continues to work EE's regular job with no restrictions. Medical treatment only.

The employee was measuring silo's and was stepping around a pipe and felt pain in the left knee.

While working on a ladder, approx. 52" high, EE experienced personal medical event causing them to fall to the ground. Co-worker notified emergency services and performed CPR until EMTs arrived. EE was pronounced DOA. Cause of death unknown at time of report.

The employee was walking down a set of stairs when employee felt a pop and pain in right knee. The employee went for medical treatment on 11/26/2018 where employee was placed on restricted duties.

The repairman was assisting a welder and was holding a piece of steel. The repairman was in a crouched position and when they stood up they felt a pop in the left knee.

While changing gudgeon bolts on the South Dribble Screw to # 2 Cooler attempted to stand from crouched position. This resulted in twisting of the employees right knee, and feeling a "pop" with pain.

Employee had been kneeling and felt a pop in left knee when ee stood up.

Employee was bending down to couple air lines for rail cars and felt pain in knee. Later the employee required physical therapy

Employee was changing a light bulb in the craneway. Material entered both eyes because of the wind. Was seen by the eye doctor. Both eyes were cleaned out. Prescribed antibiotic.

Employee bent down on right knee, picked up a 4 conductor #6 cable to check that it was the correct wire spool. As employee raised back up to standing position, employee felt a pinch in lower back and by 12:30pm employee could hardly stand up straight.

Miner was removing castable from a quencher spout and piece of castable blew into his eye.

Back of right knee sore. Cause is unknown. Doctor appointment 05/01/2013 placed on restricted duty

Pre-existing injury. Employee had injury while in the service. Had knee replacement. He was seeing the VA Hospital at Des Moines for problems he was having with the knee replacement. He had an appointment on 2/15/13 at the VA Hospital to schedule his surgery.

Removing cement chunks from withdrawal silo 55. In the kneeling position. Stood up, started to walk down the stairs, felt pop in his left knee. He felt that his knee would be ok in a couple of days. He stated he wanted to see the doctor on 6/18/12. No restrictions at or medications at that time. Dr. visit of 7/10/12 prescribed medication and therapy with no restrictions.

Employee was cleaning pre heater tower when he bent down his knee twisted

After pushing in a tire plug to repair a leak injured worker stood up and felt a pain in his lower back. Doctor has now placed him on physical therapy.

Employee was inspecting the north side of the clinker cooler. Wind swept up clinker dust behind the employees safety glasses causing an irration to the employees right eye

Employee was walking down stairs when he felt a pop or pull in his groin area.

Injured worker was kneeling on angle iron. When he got up and turned he felt pain in his right knee.

Injured worker was driving a 777 Cat Haul Truck while stripping in the quarry. He hit a rut while driving the truck causing him to bounce around in the seat. The next day he felt pain in his chest and stomach area. He reported the accident to his Supervisor on 4/6/09. He was sent to the Doctor the same day he reported the accident. He has been placed on restricted duty.

While working on iron ore screw, bent down to do some measurements, felt a "pop" in his left knee. No pain at first, later felt pain and knee filling with fluid. Went to doctor 10/10/08.

Flying debris from istalling window pane got into his left eye. Went to doctor on 7/31/08 and was treated with Tobradex drops.

Monday morning 7/14/08 injured worker felt something in his right eye. He said that it bothered him and would go away. On Tuesday morning he woke up and felt something catching on his right eye lid. On Tuesday morning reported the problem to his supervisor. He was than taken to Health Works Occupational Health.

Injured employee was found slumped over inside his haul truck with a trauma to the head. Incident is unknown at this time. Investigation continues.

Employee was working in a new clinker cooler and stated he felt some type of pain on his left leg at knee. He came to first aid and reported it. He stated nothing struck or hit his knee. Took employee to doctor. Doctor stated that it was unknown what caused it. Stated it could be a spider bite. Treated with antibiotic.

Cement dust blew into eye

IW WAS REPAIRING A DUST ARRESTOR WHEN AIRBORNE DUST BLEW INTO HIS EYES

Injured employee was riding side of moving railcar when wind gust blew open door on adjacent building. Employee thought she could fit through opening, but door caught her in the side and squeezed her against railcar. No permanent or serious injury but had severe bruising. All swinging doors were removed and replaced with roll-up doors if needed.

Injured was operating small crane when he left cab to hook up next item to be lifted. While bent over, strong wind gust swung crane line ball which struck him behind ear. He was wearing hardhat, but it struck just below rim. Was instructed to lower ball to ground to prevent swing.

EE WAS WALKING ALONG BELT 30 CATWALK WHEN THE WIND PICKED UP, CAUSING A DUST CLOUD TO FORM IN HIS PATH OF TRAVEL. PARTICLES FROM THE DUST CLOUD ENTERED EE'S LEFT EYE. IRRITATION AND TEARING RESULTED IMMEDIATELY. PRESCRIPTION AND RESTRICTIONS ISSUED.

LYING ON SIDE TO REACH WORK AREA WHERE EE WAS WIRING, STOOD UP WITH SHARP PAIN IN LOWER BACK. WENT TO DOCTOR ON 9/24/2004 AS PAIN WAS STILL THERE.

Employee walking through plant when dust got into eye.

Unloading water jugs from raw mill patio to take into shift lunchroom when he was bitten on left hand. at 5:30 pm hand was swollen and he was unable to make a fist without pain.

EE did not have an additional accident but aggrevation to back due to previous injury dated 6-16-02.

AS INJURED EMPLOYEE WAS SHOVELING CLINKER DUST INTO A HOPPER, WIND BLEW DUST INTO HIS RIGHT EYE.

INJURED EE WAS DOING INSPECTIONS WHEN THE WIND BLEW ROAD DUST INTO HIS RIGHT EYE, CAUSING A CORNEAL ABRASION.

INJURED EE WAS WORKING IN WINDY AND DUSTY CONDITIONS ALL DAY. AT THE END OF THE WORK DAY, HE FELT SOMETHING IN HIS LEFT EYE. THE FOLLOWING MORNING, HIS EYE WAS SWOLLEN.

CLEANING BUILD UP OFF OLD PACKHOUSE ROOF WHEN TWO HIVES WERE DISTURBED AND THE BEES CAME AFTER HIM.

INSPECTING FORE EXTINGUISHER AROUND TIRE SINGULATOR WHEN A SWARM OF WASPS ATTCKED HIM. STUNG ON NOSE, ARM, TORSO APP 65 TIMES. FACE SWELLED UP AND HE WAS TAKEN TO HOSPITAL.
