The employee was working on the vent dust collector and was going from hopper to hopper and was standing on metal grating. When EE turned to move EE's foot stayed where it was as EE continued to twist and EE heard a pop in EE's knee.

Operator trainee reported to supervisor EE was reaching overhead and felt a sharp pain in EE's left shoulder. Employee thought it might be a re-occurrence of an earlier injury sustained in Nov of 2019. Employee was referred to clinic, who recommended 2wks of physical therapy. Employee continues to work EE's regular job with no restrictions. Medical treatment only.

While working on a ladder, approx. 52" high, EE experienced personal medical event causing them to fall to the ground. Co-worker notified emergency services and performed CPR until EMTs arrived. EE was pronounced DOA. Cause of death unknown at time of report.

The repairman was assisting a welder and was holding a piece of steel. The repairman was in a crouched position and when they stood up they felt a pop in the left knee.

While changing gudgeon bolts on the South Dribble Screw to # 2 Cooler attempted to stand from crouched position. This resulted in twisting of the employees right knee, and feeling a "pop" with pain.

Employee bent down on right knee, picked up a 4 conductor #6 cable to check that it was the correct wire spool. As employee raised back up to standing position, employee felt a pinch in lower back and by 12:30pm employee could hardly stand up straight.

Pre-existing injury. Employee had injury while in the service. Had knee replacement. He was seeing the VA Hospital at Des Moines for problems he was having with the knee replacement. He had an appointment on 2/15/13 at the VA Hospital to schedule his surgery.

Removing cement chunks from withdrawal silo 55. In the kneeling position. Stood up, started to walk down the stairs, felt pop in his left knee. He felt that his knee would be ok in a couple of days. He stated he wanted to see the doctor on 6/18/12. No restrictions at or medications at that time. Dr. visit of 7/10/12 prescribed medication and therapy with no restrictions.

Injured worker was driving a 777 Cat Haul Truck while stripping in the quarry. He hit a rut while driving the truck causing him to bounce around in the seat. The next day he felt pain in his chest and stomach area. He reported the accident to his Supervisor on 4/6/09. He was sent to the Doctor the same day he reported the accident. He has been placed on restricted duty.

Monday morning 7/14/08 injured worker felt something in his right eye. He said that it bothered him and would go away. On Tuesday morning he woke up and felt something catching on his right eye lid. On Tuesday morning reported the problem to his supervisor. He was than taken to Health Works Occupational Health.

Employee was working in a new clinker cooler and stated he felt some type of pain on his left leg at knee. He came to first aid and reported it. He stated nothing struck or hit his knee. Took employee to doctor. Doctor stated that it was unknown what caused it. Stated it could be a spider bite. Treated with antibiotic.

Injured employee was riding side of moving railcar when wind gust blew open door on adjacent building. Employee thought she could fit through opening, but door caught her in the side and squeezed her against railcar. No permanent or serious injury but had severe bruising. All swinging doors were removed and replaced with roll-up doors if needed.

Injured was operating small crane when he left cab to hook up next item to be lifted. While bent over, strong wind gust swung crane line ball which struck him behind ear. He was wearing hardhat, but it struck just below rim. Was instructed to lower ball to ground to prevent swing.

EE WAS WALKING ALONG BELT 30 CATWALK WHEN THE WIND PICKED UP, CAUSING A DUST CLOUD TO FORM IN HIS PATH OF TRAVEL. PARTICLES FROM THE DUST CLOUD ENTERED EE'S LEFT EYE. IRRITATION AND TEARING RESULTED IMMEDIATELY. PRESCRIPTION AND RESTRICTIONS ISSUED.
