Mechanic was working on a wiring problem on the front engine of a 637 Scraper. A strong gust of wind blew the engine hood down on the mechanic, who was standing in front of the equipment cab. The hood pushed the mechanic's face onto the small side door cutting his nose. The injury was treated @ clinic, stitches were used to repair the cut. No work restrictions or lost time.

Employee was sitting down at main plant packer operating packer. Employee turned to reach behind him for empty bag to place on packer spout, as he turned, he pulled a muscle in his back. Employee went to ER in Spearfish 11-19-06 where he was treated for low thoracic sprain.

AFTER PLACING LOCKING SEAL ON BULK RAILCAR, INJURED WAS EXITING FROM UNDER RAILCAR, WHEN WIND BLEW DUST UNDER HIS SAFETY GLASSES INTO HIS RT. EYE, HE CLEANED HIS EYE OUT ON 7/11, THOUGHT IT WA S O-K, THEN BOTHERED HIM LATER, SO HAD EYE DR. CHECK IT.

AFTER SPRINTING APPROX 300 YARDS TO RETRIEVE AN OXYGEN BOTTLE TO ASSIST A FELLOW EMPLOYEE THAT WAS FEELING SICK AND DIZZY. UPON SEVERAL MINUTES OF OBSERVING THE ILL EMPLOYEE, HE LOST CONSCIOUS NESS. AN AMBULANCE WAS SUMMONED AND HE WAS TAKENTO HOSPITAL.

STUNG BY WASP WHILE WORKING IN HOLEPLUG BUILDINGPUT COLD PACK ON HAND. WORKED NEXT DAY. AT END OF SHIFT ON 9/19 HAND WAS SWOLLEN WITH INFECTION.HAND WAS OK AT MIDDLE OF SHIFT.

EMPLOYEE FELT TWINGE IN BACK ON 3-21-01 WHILE WALKING FROM WORK AREA. THE NEXT MORNING, HE WAS STANDING IN LUNCHROOM WHEN HE COUGHED AND HIS BACK SPASMED. EE WENT TO DR WHO GAVE HIM SOME MUSCL E MEDICATION AND ADVISED HIM HE COULD RETURN TO WORK WHEN HE FELT BETTER. HE WENT BACK TO WORK ON THE NEXT SCHEDULED SHIFT.
