EMPLOYEE WAS FOUND UNCONSCIOUS IN THE CRUSHER BUILDING ON THE 2ND FLOOR NEAR THE STEPS GOING TO THE THIRD FLOOR. NO INJURIES WERE VISIBLE. DURING THE PAST NINE YEARS, HE HAS SUFFERED BLACKOUTS AND CONVULSIONS APPARENTLY AS A RESULT OF AN AUTOMOBILE ACCIDENT.

EMPLOYEE HAD EXITED THE DOZER AND WAS STANDING ON THE PUSH BAR WITH HIS LEFT HAND ON TRHE GRAB RAIL NEAR THE SIDE OF DOOR. THE WIND BLEW THE DOOR SHUT. THE DOOR CAUGHT THE EMPLOYEES GLOVE AND PULLED HIS LEFT INDEX FINGER INTO THE DOOR FRAME MASHING THE FINGER.
