The employee had finished fueling a machine and was driving fuel truck away and stalled the engine. He rolled backward approximately 10 feet and hit the track of the machine. Employee pulled muscles in right shoulder.

Showing all 7The rock truck driver backed up to the trackhoe to be loaded. The spoil rock on the left side of the rock truck slid down against the truck wheels. The action jarred the truck. The driver claimed his back was hurting, and was taken to the emergency room for examination.
Employee was traveling on haul road, missed gear and lost control of water truck and rolled truck over on side.
Employee was returning to pit and lost control of rock truck and hit head on back of cab.
EE stated he lowered stands on coal crusher and then attempted to pull the fifth wheel release lever. He stated the release lever was hard to pull and he felt something tear in his neck.
EE was repalcing area differential in service truck and when he stretched to guide a part into place he strained his neck and upper back. He did not lose any time and did not seek medical treatment. He did not report injury to mine mgt. until 1/3/06. On 5/3/06 He notified mgt he would be off work beginning 5/4/06 with surgery for 2 pinched nerves and one disk replacement.
Employee was returning to the trackhoe when he ran over a rough place in the road causing him to bounce into the roof of the truck. He told his supervisor that he did not have on his seat belt.
