The plant was off for maintenance. Employee was greasing a bearing on the BCV155 conveyor head pulley. The plant operator turned on the plant, the start up alarm sounded. Employee did not lock out / tag out the conveyor. The conveyor started and pushed him into a safety pole that holds safety cables around the conveyor.

Showing all 2The haul truck operator either fell asleep or was not paying attention and drove over the 3 foot safety berm and landed on the next level of the pit. The truck landed on the front left side of the haul truck. The operator exited the truck by himself and a coworker stopped and notified the supervisor. The weather was sunny, 90F degrees, no fog and no mud on the haul road.
