Employee was operating haul truck #2 going down east deep ramp. While traveling down the ramp the truck hit a bump in the roadway. Employee felt pain in lower back. Reported to the onsite clinic and self-managed onsite until Monday (06/04/18) when the employee was seen by physician in Fairbanks whom placed employee on restricted/modified duty.

Showing all 5Operator and helper were driving down ramp in the 1300EXP on a cat 414E tractor, when they lost their brakes.The operator attempted to down shift and use the parking brake and was unable to complete the downshift or slow the tractor down,running into the rib.
Concrete boom pump departing mine on road back to highway, going down steep hill, operator heard loud bang from the drive line and began free-wheel. Waiting too long to brake, gained too much speed to slow with air brakes. Operator picked best place by driving off the road at a spot which it crossed ditch and headed back up the hill side.
Helicopter crash resulting in one fatality. This incident is currently being investigated by the Operator, FAA, & NTSB so their reports are forthcoming.
While standing on vehicle that was being lifted by another, the vehicle dropped approximately 3 to 4 feet. This didn't become reportable until August 9, 2012 when employee started missing.
