Employee attempted to dump a self dumping hopper into the grizzly. The lever on the hopper was not releasing, so employee held the lever down with left hand and pushed the hopper so it would dump. When the hopper dumped, it bounced and EE's left hand on the handle was pinched on a bolt on the forklift.

Employee bumped by a forklift load of 50lb bags of product while taping cardboard inside a trailer. Employee fell backwards and the following day was diagnosed with sprain of ligaments of thoracic and lumbar spine. Employee received lifting restrictions.

Employee was driving on mine road and felt strain in neck muscle.

Employee states being towed through the mud at mine site (after getting stuck in mud) created jerking motions which resulted in shoulder, rib, neck, and back pain. Pain could also possibly be due to repetitive motion involved in assigned duties of driving. Driver first reported to Employer on 2/17/14.

Employee was in the loader, digging into the stockpile, felt his lower back pop down low and get tight. Employee noticed paid and pulling down his right leg when leaving the loader. Employee was diagnosed with a lumbar strain and was issued work restrictions.

Employee was getting on and off the forklift and began to experience groin pain.

Employee was stepping up onto a forklift when he felt a pain in his side.
