Showing all 6The employee was entering the warehouse driveway and a truck was in the driveway waiting in line to be loaded. The worker pulled to the left of the truck and made a hard left turn toward the parking area and hit a light pole. The light on the light pole was on at the time of the event. The worker complained of neck and back pain.
The employee was washing the tracks and was called on the radio and notified that the railcars were being moved to stand clear. After the 1st & 2nd car passed the employee was snagged by the 3rd car and thrown to the ground breaking EE's L knee cap.

EMP DRIVING MANLIFT WHEN REAR WHEEL FELL INTO A HOLE CAUSING EMP TO BE THROWN DOWN INTO BASKET INJURING RIGHT KNEE. REPORTED BUT DID NOT GO TO DR AT THAT TIME. AS TIME PASSED, KNEE KEPT GETTING WORSE. HE DECIDED TO SEE DR AFTER A PERIOD OF TIME AND ULTIMATELY HAD SURGERY RESULTING IN TIME LOST.
