Employee was working out of a 45' man lift. EE was bolting up purlins. EE had to move the basket around the column to complete bolt up. When EE moved the basket it lurched to EE's left catching EE's finger between the column and the basket.

Showing all 3While tramming an electric locomotive, out of the mine, dust, dirt or another unknown debris entered the miner's eye. The event resulted in an abrasion to the eye and antibiotics were administered to the eye.
An OSMI foreman was driving an electric locomotive into the mine and upon approaching a run around, the foreman reached down for a bottle of water. At the moment they sat up, the foreman fell from the locomotive and was crushed between the locomotive and rib. The foreman suffered broken ribs and a fractured pelvis.
