The employee was driving a utility cart back to the third bank silos. Visibilty was reduced due to weather conditions. The employee hit the vertical stand of a truck wash. The employee was injured due to the impact.

While vacuum cleaning interior of the refractory line duct an employee's hand became lodged in vacuum hose. The truck was shut down and the employee evaluated. Corrective Action: Vacuum breaker and hose handle to be installed on all vacuum operations involving personnel handling the flex hose.
Employee was operating rail car mover with cars attached, could not stop mover or cars, struck rail car parked on siding. Extensive damage to mover and injuries to employee. MSHA was notified immediately, investigation in progress.
Employee went directly to coal screw area at start of shift to ensure no blockages. Within a couple minutes of arriving at coal screw, employee radioed that ee needed help. Supervisor found ee laying on the grating stating that ee had injured hand. Employee had lost the tip of one finger and lacerated two others. Employee stated ee didn't know how it happened.
As the employee was riding on a rail car, EE observed a forward car to begin derailing. Upon seeing the forward car derail; the employee jumped off of the car EE was riding. EE injured knee upon landing causing a work restriction.
