Truck was in gear, lurched forward, collided with dumpster and slammed driver's door into employee's body. Employee received a laceration to upper lip, requiring stitches at local ER.
Contractor was unloading dirt from Hannah Mine #1175 at Specialty Vermiculite where the box tipped over on the right side. It is believed that contractor driver attempted to dump on uneven soft ground. Accident resulted in minor chest injuries to contractor driver. Chest wall contusions, 7th rib, physical damage to trailer box.
TRUCK OPERATED BY CONTRACTOR WAS DELIVERING LOAD TO MINE. DRIVER DID NOT SEE TRAIN. TRAIN STRUCK POWER UNIT ON DRIVER SIDE. DE-RAILED ENGINE AND 2 TANK CARS, DRIVER'S WIFE TOOK DRIVER TO HOSPITAL AFTER SCENE CLEARED TO BE CHECKED, WAS TREATED AND RELEASED TO FULL DUTY FOR THE FOLLOWING DAY.
