Employee was welding on a chute at the stoker loadout. Employee was in a basket attached to a CAT telehandler. The basket was raised six feet off the ground when it unattached from the telehandler falling to the ground. Employee sustained a fracture to the left tibia.

Showing all 3Helping change ram car batteries. The safety chains were tight, so the car pushed the battery into the rib to take tension off the chains. I.E. reached in to loosen the chains when they became tight again pinching their right index finger between the battery and the safety chain. Caused a fracture and laceration that required stitches to the I.E's right index finger.
Employee was helping another start the face scoop. Employee sat down in the cab and leaned forward to pull on the breaker handle when employee felt a pain in lower back. Through company investigation, information received January 6th, 2023 it was determined this incident need be reported.
