Victim stated he was run off road by oncoming truck causing his loaded truck to upset. Heavy damage to the truck. At time of accident roadway was clear and dry.
EE had parked his service truck in order to work on a grease truck. Although the park brake had been set, the service truck began to roll backwards. He was caught between the driver's door of the service truck and the front bumper of the grease truck.

OPERATING ROCK TRUCK, RAN OVER ROCK CAUSING HIM TO TWIST HIS BACK. REPORT FILED LATE DUE TO PROBLEMS OBTAINING MEDICAL INFORMATION.
EE WAS OPERATING SCOOP CAR, HE RAN OVER 6'ROOF BOLTS AND BOLT FLEW UP AND STRUCK HIM IN THE FOREHEAD. COMPLIANCE NOT A FACTOR.
EE WAS HELPING OTHERS TO LOAD FLINGER DUSTER ONTO THE BUCKET OF THE SCOOP. THE INJURED ( NOT REALIZING WHERE HIS FOOT WAS) TOLD THE OPERATOR TO LOWER THE SCOOP BUCKET. THE OPERATOR LOWERED THE BUCKET & PINCHED HIS FOOT BETWEEN THE BUCKET & THE MINE FLOOR. INATTENTION TO ACTIVITY A FACTOR.
HE ALLEGES THAT HIS HEAD STRUCK THE MINE ROOF WHILE RIDING INSIDE ON A 3 WHEELER PERSONAL CARRIER.
EE & CO-WORKER ARRIVED IN THE PIT TO REPAIR A LOOSE BUCKET TOOTH RETAINING PIN ON A P&H BUCKET. THEY PARKED THE SERVICE TRUCK PARALLEL TO THE BUCKET. INJURED WAS PERFORMING THE WELDING, HE WAS KNEELING IN FRONT OF THE BUCKET. A HAUL TRUCK WAS PARKED APPROX. 200'FROM THE BOTTOM OF THE RAMP. IT BEGAN MOVING, STRIKING THE SVC.TRUCK THENJACKKNIFING AND STRIKING EE AND THE BUCKET.
HE WAS TRAVELING ON A 3 WHEEK PERSONNEL CARRIER, WHEN HIS HARD HAT FELL OFF AND HE STRUCK HIS HEAD ON THE MINE ROOF.
EMPLOYEE WAS IN THE ENTRY OUT BY END OF A MANTRIP, TRAVELING TOWARD THE ELEVATOR AT THE END OF HIS SHIFT. ON THE SOUTH MAINS TRACKS, THE MANTRIP COLLIDED WITH A 15 TON JEFFREY MOTOR AND ITS BA TTERY CAR. THIS RESULTED INTO HIM RECEIVING A HARD JOLT, INJURING HIS NECK. THIS BECAME A LOST TIME INJURY ON 4-25-01.
EMPLOYEE STARTED CROSSING TRACK WHILE JEEP WAS BACKING UP AND JEEP HIT HIM KNOCKING HIM TO GROUND, FRACTURE LEFT TIBIA.
EE WAS OPERATING A MANTRIP ON THE 3R3NE TRACK. A SET OF BELT CLAMPS WERE STICKING OUT THE SIDE OF THE MANTRIP. THE CLAMPS HIT A CRIB ALONG THE TRACK, DISLODGING IT. THE CRIB BLOCKS FELL, STRIK ING EE ON THE RIGHT KNEE.
EE WAS UNLOADING SUPPLIES AT THE C-3 HAND TEAR DOWN AREA. HE WAS IN THE BUCKET OF A SCOOP HOOKING A CHAIN TO SCOOP'S RAM. AFTER THE CHAIN WAS HOOKED HE PLACED HIS RIGHT HAND ON THE RAM. WHEN THE RAM WAS PULLED BACK THREE OF HIS FINGERTIPS WERE PINCHED. THE TIP OF HIS MIDDLE FINGER WAS FRACTURED.
EE WAS OPERATING A 15 TON MOTOR, AND HE HAD REDUCED THE SPEED OF THAT MOTOR AS IT APPROACHED A TRACK SWITCH. THE OPERATOR OF ANOTHER MOTOR, FOLLOWING EE S MOTOR FAILED TO SLOW DOWN FOR THAT TR ACK SWITCH, AND COLLIDED INTO THE REAR OF EE'S MOTOR. THIS KNOCKED EE INTO OPERATOR DECK FRAME, FRACTURING TWO OF HIS RIBS. NO REPORT WAS MADE OF THIS INJURY UNTIL 12-1-00.
RIDING OUTSIDE ON JEEP. SWITCH WAS LEFT OPEN TO PUT EMPTY AWAY AT #6 PASSWAY. JEEP WENT INTO SWITCH AND HIT MOTOR. INJURED WAS THROWN AGAINST JEEP INJURING LEFT HIP AND SHOULDER.
EE WAS OPERATING A JEFFREY 15 TON LOCOMOTIVE COMING OUTBY ON THE 3R3NE TRACK. THE LOCOMOTIVE DERAILED ON A HILL AND SLID BACKWARDS DISLODGING TWO TIMBERS. EE TWISTED HIS BODY TO PUSH ONE OF TH E TIMBERS OFF OF THE MOTOR AND STRAINED A MUSCLEIN HIS LOWER BACK.
EE WAS A PASSENGER IN A MANTRIP TRAVELING TO THE C-2 LONGWALL PANEL. WHEN THE MANTRIP ARRIVED AT THE 8 LEFT TRACK SWITCH IT DERAILED AND IT LANDED ON THE MINE FLOOR. THE IMPACT CAUSED A SUDDEN PAIN IN HIS NECK AND BACK. HE IS SCHEDULED TO BEGIN A SERIES OF PHYSICAL THERAPY TREATMENTS FOR THIS INJURY.
WHILE TRAVELING OUT OF THE MINE THE TROLLEY POLE#23 MAN BUS CAME OFF THE WIRE. THE POLE BASE LIFTED UP ALLOWING THE POLE TO COME DOWN AND STRIKETHE EMPLOYEE ON THE LEFT SHOULDER AND ARM.
EE WAS DRIVING A PICKUP TRUCK. HIS LUNCH BUCKET STARTED TO FALL OFF THE SEAT AND WHEN HE REACHED FOR IT HE RAN IN THE DITCH HITTING HIS RIGHT HAND ON THE DASHBOARD. X-RAYS REVEALED A SLIGHT HA IRLINE FRACTURE IN HIS MIDDLE KNUCKLE, RIGHT HAND.
