OPERATOR OPENED3LID OF CABLE REEL TO OBSERVE AMT OF CABLE LEFT ON REEL.WHILE LID WAS OPEN HE ASKED INJ TO CHECK CABLE AFTER HE HAD TAKEN TORKAR PAST INTERSECTION.UNAWARE INJ PLACED HAND ON FRA ME OF TORKAR,OP9RATOR MOVED TORKAR FORWARD AND DOOR CLOSED ON INJ RIGHT HAND.
THE INJURED HAD3BLOCKED AND UNCOUPLED 2 MINE CARS FROM A TRIP THE 2 MINE CARS BEGAN TO ROLL IN THE DIRECTION OPPOSITE BLOCKED THE INJ STARTED WALKING ALONG SIDE THE MOVING CARS ATTEMPTING TO BLOCK THEM A8 HE WAS DOING THIS HE BECAME CAUGHT BETWEEN THE CAR AND AN INSTALLED POST
THE OFF STD TOR5AR WAS INT H E PROCESS OF TRAMMING TO T HE RAMP APPROX 100' OUTBY THE RAMP HE RAISED THE BOOM WHILE TRAMMING. WHEN HE APPROACHED THE RAMP HE STOPPED TO BE SURE BOT TO CATCH THE RAMP WITH 7HE DRAG. AS THE TORKAR WAS STOPPED THE FOREMAN WHO WAS ON THE RT SIDE OF T HE RAMP & OUT OF SIGHT STEPPED ONTO THE RAM THE TORKAR WAS THEN TRAMMED SLOWLY PINNING INJURED.
THE INJURED WAS WALKING ALONG THE SHUTTLE CAR TRAM ROAD AND HAD STEPPED ACROSS A SHUTTLE CAR TRAILING CABLE WHEN THE PIECE OF EQUIP. STARTED. THE TRAILING CABLE PICKED UP BY THE REEL CAUSED TH E CABLE TO STRIKE THE INJURED UNDER THE CHIN CAUSING A LACERATION.
WHILE PULLING ROAD CLEANER THRU SWITCH AND AROUND CURVE MOTORMAN SLOWED DOWN SWIFT BEHIND ROAD CLEANER TURNED TO LOOK IF HARD WAS CHANGING WIRE RAN INTO BOOM OF ROAD CLEANER.
RIDING IN JEEP-PERSONNEL CARRIER-JEEP SLID INTO LOCOMOTIVE PARKED AHEAD OF IT ON TRACK.
INJ PASSENGER ON JEEP.WHILE IN MOTION,JEEP STRUCK BROKEN RAIL AND INJ WAS THROWN AGAINST FRONT OF JEEP STRIKING HIS FACE.HE REC\D BRUISES OF THE FACE.
EMPLOYEE WAS BETWEEN LOADS CUTTING LOADS WHEN CARS MOVED CAUSING HIM TO TWIST HIS SHOULDERS.
PULLING LOADED MINE CARS WITH A JEFFREY 37 TON MOTOR POLE CAME OFF TROLLEY WIRE INJURED REACHED FOR POLE WITH LEFT HAND AND POLE SPRUNG UP AGAINST ROOF PINCHING LEFT HAND BETWEEN POLE AND MIN E ROOF. INJURED SHOULD HAD USED AIR ACTUATED POLE RETREAVER. INJURY CONSISTS OF A FRACTURE LEFT MIDDLE FINGER FIRST JOINT.
THE INJURED WAS OPERATING A S/C OUTBY THE SECTION. THE OP CANOPY HAD BEEN REMOVED DUE TO HEIGHT RESTRICTIONS. HE TRAMMED THE S/C ONTO SOME SLOUGHED RIB. THIS REDUCED THE HEIGHT & AS THE S/C W AS RAISED HIGHER THE OP WAS FORCED INTO A TIMBER RESULTING IN BRUISES TO HIS CHEST & BACK AS HE WAS SQUEEZED.
THE INJURED WAS RIDING IN A MINE JEP FACING THE DIRECTION OF TRAVEL. WHEN THE JEEP CROSSED A SWITCH A FAULTY AXLE JOURNEL HAD DROPPED BELOW THE JEEP FRAME AND SAME STRUCK THE TRACK SWITCH FROG . THE JEEP STOPPED SUDDENLY AND THE INJURED WAS THROWN FORWARD STRIKING HIS CHIN ON THE FRONT OF THE JEEP.
EMPLOYEE WAS CUTTING EMPTIES 4 EMPTIES SKIDDED AND PINCHED FINGERS BETWEEN CUT OFF AND WAGONS
INJURED WAS REPLACING TRAM CHAIN ON JEEP WHEN JEEP ROLLED FORWARD CATCHING HIS RIGHT INDEX FINGER BETWEEN SPROCKET AND CHAIN.
JEEP RAN INTO A TRIP OF ON IT COAL CARS.
EMPLOYEE WAS RIDING IN A PORTA BUS (MANSTRIP)HISCOAT WAS DRAGGING SO EMPLOYEE REACHED OUT TO PULL IT IN THERE WAS SOME ICE BUILD UP AND EMPLOYEE PINCHED LT.ARM.
EMPLOYEE WAS PUSHING MT\S BACK IN HOLE WITH RAM MOTOR-AFTER DOING SO EMPLOYEE PULLED MOTOR BACK AND RAM ARM WAS NOT FULL RETRACKED AND CAUGHT BETWEEN WAGONS CASUSING BRAKE WHEEL TO SPIN VIOL ENTY TWISTING WRIST.
TRYING TO HOLD S.C. SNUB WHILE S.C. WAS TRAMMING TOWARDS TIPPLE. GOT HAND CAUGHT BETWEEN SNUB & POST ON BACK OF ROOF BOLTER.
EMPLOYEE WAS OPERATING A MOTOR THE POLE CAME OFF THE WIRE IN REACHING FOR THE POLE EMPLOYEE STRUCK HAND AGAINST BEAM
HAD RIGHT FOOT ON RAIL WHILE UNCOUPLING WAGONS.WAGONS DIDNT UNCOUPLE AND SUPPLY WAGON RAN OVER EMPL RIGHT FOOT.
