EMPLOYEE SAW A 8TEEL PLATE ON THE RAW COAL BELT AND TRIED TO LIFT IT OFF.THE PLATE CAUGHT ON THE BELT AND SQUASHED HIS MIDDLE FINGER.IN THE FUTURE THIS EMPLOYEE WILL SHUT OFF THE BELT.HE RE QUIRED SUTUES A3D A SPLINT BUT QWAS ABLE TO RETURN TO HIS NEXT SHIFT AND PERFORM REGULAR DUTIES.

WE WERE FIRST A4ARE OF THIS ACCIDENT ON 11 17 86 AND FIRS DAY MISSED WAS 11 20 86 EMP TWISTED HIS BACK AS HSI TRUCK HIT A BUMP DRIVING OUT OF THE ROCK TUNNEL RECURENT BACK PAINS AND SPASM HAS CAUSED HIM TO M3SS WORK

EMPLOYEE WAS A 1ASSENGER IN ONEIMCO PERSONNEL CARRIER.THE VEHICLE HIT A BUMP AND I NAME STRUCK AGAINST THE SIDE OF THE EIMCO.HE MISSED 2 DAYS RO RECOVER FROM BRUISED MUSCLES.

MINER OPERATOR 9UNNING 12 CM BY REMOTE IN UNSAFE POSITION ALONGSIDE MACHINE IN VIOLATION OF COMPANY SAFETY RULES.HE WAS ON DOWNHILL SIDE AT TIME AND APPARENTLY SLID ON MUDDY GROUND AND PINNED HIM AGAINST TIM6ER.INCURRED ABDOMINAL/PELVIC TRAUMA REQ SURGERY AND IS STILL HOSPITALIZED.
EMPLOYEE WAS DR3VING THIS TRUCK TO THE CAR WASH WHEN ATTEMPTED TO DOWNSHIFT THE GEARBOX. HE COULD NOT GET THE TRUCK BACK IN GEAR AS IT WAS GAINING SPEED. HE TRIED THE BRAKES REPEATEDLY, BUT THEY FAILED TO 2PERATE. HE JUMPED CLEAR AND HIT HIS HEAD ON THE GROUND.

WHILE CHAINING 6P TENSIONER TO BELT DRIVE THE BELT DRIVE WA TURNED ON AND BEFORE I.NAME COULD GET HIS HANDS OUT, THE CHAIN PINCHED IT AGAINST A FRAM. HE INCURRED AN OPEN CRUSH INJURY, WHICH R EQUIRED SUTURIN1 HE REPORTED FOR WORK THE FOLLOWING SHIFT
EMPLOYEE WAS RE1AIRING A SWITCH TO A STRIKE BELT WHILE ANOTHER MAN WENT TO SHUT OFF POWER HE GOT ATARTED HE INADVERTENTLY STEPPED ON THE BELT WHILE HOLDING THE ENDS TO THE SWITCH LINE HE HAD N OT CONFIRMED TH5T THE POWER WAS OFF THE LEADS CAME INTO CONTACT AND STARTED THE BELT WHICH PULLED HIM INTO A CROSSOVER AND KNOCKED HIM TO THE GROUND

WHILE DRIVING SHUTTLE CAR EMPLOYEE STATED HE FELT PAIN IN HIS GROIN. AND RIGHT TESTICLE. THIS WAS LATER FOUND TO BE DUE TO A HERNIA EMPLOYEE HAD AN OPERATION ON 9-25-85 AND WILL BE OFF AN ESTI MATED 6 WEEKS.
EMP WAS RIDING TO THE COMPANY BATHHOUSE IN CO. BUS THE BUS RAN INTO SOME DEEP RUTS & BUMPS CAUSING MEN IN THE BACK OF THE BUS TO BUMP THEIR HEADS. WHEN FIRST REPORTED THIS EMPLOYEE DENIED HE WOULD BE WANTING MEDICAL ATTENTION. AFTER MISSING WORK A FEW DAYS LATER WE LEARNED THIS WAS DUE TO SOME PRE-EXISTING BACK PROBLEM AGGRAVATED BY THE INCIDENT

WHILE PUSHING MANTRIP RAIL CAR BACK ONTO THE RAILS AFTER IT RODE ONTOP OF THE DEADSTOP TIE AT THE END OF THE TRACKS, THE TIE BROKE AND THE CAR ROLLED BACK INTO THE EMPLOYEE AS HE TURNED AWAY I T PUSHED AND TWISTED HIS KNEE.

EMPLOYEE LIFTED A ROCK ONTO THE CONVEYOR THAT FEEDS INTO THE ROCK CRUSHER AND CAUGHT HIS RT FINGER BENEATH TEH ROCK AND MASHED IT AGAINST ANOTHER ROCK ON THE CONVEYOR A LACERATION AND MINOR F RACTURE OF THE DISTAL TUFT OF THAT FINGER WERE SUSTAINED REQUIRING SUTURES
WHILE OPERATING 10SC CAR OVER SOME BUMPS AT A HIGH RATE OF SPEED. THE OPERATOR WAS BOUNCED AROUND IN THE CAB OF HIS SHUTTLE CAR AND SPRAINED HIS LOWER BACK. THE EMPLOYEE IS MISSING WORK PENDIN G FURTHER TESTS.
WHILE FOREMAN WAS LOOKING FOR MISFIRES AT THE TUNNEL FACE AN EIMCO LOADER UP HE TURNED THE BUCKET TOWARD I,NAME AND COULD NOT SEE HE WAS SQUEEZING HIM AGAINST THE RIB. I,NAME SUSTAINED NUMEROU S INJURIES CAUSING A LOST TIME ACCIDENT.
THE FIREBOSS WAS RIDING ON THE BRAKE CAR WITH THE ROPE-RIDER COMING UPHILL ON THE MAN-TRIP. THE TRIP DERAILED AND THE ROPE-RIDER MOVED, TO BELL THE HOIST TO STOP THE TRIP ACCDIENTLY HITTING TH E FIRE BOSS\S FACE IN THE MOTION. THE ROPE RIDER HAS BEEN COUNSETTED ON HOW TO HOLD THE RAPPER IN THE CORRCT HAND TO AVOID TURNING TO REACH THE WIRES.

THE OPERATOR WAS DEPENDING ON JOHNS SIGNALS AS HIS IVISBILITY WAS BADLY RESTRICTED HE KEPT THE BUCKE T LOW SO HE COULD SEE HIM. AS HE DROVE FORWARD, ON I,NAME SIGNAL, THE BUCKET BOTTOMED OUT ON TOP OF HIS RIGHT FOOT BECAUSE OF THE UNEVEN GROUND. HE INCURRED A FRACTURED FOOT.
AS THE CREW WAS DEPARTING THE MINE ON THE HOIST OPERATED MANTRIP, THIS EMPLOYEE LEANED AGAINST TXX AN IMPROPERLY INSTALLED GUARD RAIL WHICH SLIPPED OUT OF ITS HOLES. THE MAN FELL OUT AND FELL BENEATH THE EDGE OF THE CAR WHICH SQUEEZED HIS BODY BETWEEN IT AND THE GROUND, HE SUFFERED VARIOUS CONTUSIONS AND WAS KEPT A DAY IN THE HOSPITAL FOR OBSERVATION ONLY
FOREMAN & SOME OF HIS CREW WERE SHOVELING TO CONTROL SPILLAGE AT THE TAILPIECE. AS HE BENT DOWN TO SHOVEL SOME COAL AN ABJECT FLEW FROM THE TAILPIECE & STRUCK THE LEFT SIDE OF HIS FACE. HE WOR KED THE FULL SHIFT & LATE THAT EVENING NOTICED A DEPRESSION ON HIS FACE-LATER DIAGNOSED AS A FRACTURED CHEEKBONE. THE REQUIRED SURGICAL RECONSTRUCTION & FOR THIS EE IS OFF WORK.
