WHILE OPERATING A 21 BUGGIE A PIEC EOF TIE WIRE WAS HANGING FROM A ROOF BOLT AND EE GOT IT IN HIS EYE.
A ROCK HUNG IN BLET CHUTE AT #2 DRIVE. EMPLOYEE SAID HE WAS UNHANGING ROCK WITH A #1 BELT RUNNING. (EMPLOYEE WAS REMOVING ROCK FROM SIDE OF CHUTE WHEN A ROCK SLIPPED AND PULLED HIS BACK. (ALLEDGED ACCIDENT)
HE WAS CROSSING A RAILROAD TRACK WHEN RACE FORK COAL CORP. WAS PUSHING CARS DOWN THE TRACK. IT WAS PITCH DARK, NO LIGHTS WERE ON THE CARS OR THEIR WERE NO SIGNS TO TELL YOU TO WATCH. THEY HIT THE TRUCK AND PUSHED IT DOWN THE TRACK.
EMPLOYEE WAS DRIVING A SHUTTLE CAR, WHEN HE HIT A ROUGH PLACE IN THE HAUL ROAD. THIS CAUSED HIM TO BOUNCE IN HIS SEAT AND JAR HIS BACK.
PERSON WAS OPERATING A RUBBER TIRED UTIITY VEHICLE COMING DOWN FROM SECTION, WHEN HE APPROACHED END OF TRACK HE TRIED TO STRADDLE THE RAIL AND THE FRONT OF VEHICLE STRUCK THE END OF THE RAIL C AUSING PERSON TO BE THROWN FORWARD AND HIS NOSE STRUCK STEERING WHEEL.
EMPLOYEE WAS DRIVING A DIESEL VEHICLE UP AN APPROXIMATE 10% GRADE WHEN HE ENCOUNTERED A BUMP (ROUGH PLACE) APPARENTLY CAUSING HIM TO HIT HIS HEAD ON A HAND RAIL OF THE VEHICLE. THIS INCIDENT W AS INVESTIGATED BY MSHA AND KDMM.
EMP WAS USING THE CASE SKIDSTEER (BOBCAT) TO PULL AN OIL TANK TO THE PLANT. AS HE WAS BACKING TOTHE PLANT HE HIT A ROUGH PLACE CAUSING THE SKIDSTEER TO BOUNCE. HE APPARENTLY TURNED THE CONTROL & DROPPED (LOWERED) THE FORKS OF THE SKIDSTEER CATCHING HIS FOOT BETWEEN THE FORK MECHANISM AND THE OPERATOR CAB.
EMPLOYEE ASSISTING IN THE TRANSPORTATION OF TIMBERS WHEN PARTICLES FROM THE TIMBERS BLEW IN THE EMPLOYEE'S FACE. THE ACCIDENT REPORT WAS NOT FILED WITH ISLAND FORK CONSTRUCTION UNTIL 2-19-96.
EMPLOYEE WAS ONA GOLF CART TRAVELING IN THE TRACK ENTRY. WENT THROUGH A CHECK SURTAIN AND RAN INTO A PARKED SUPPLY TRAILER THAT HAD BEEN BROUGHTTO THE TRACK TO BE PUT ON THE RAIL.
PERSON WAS BACKING A SECTION LUBE TRUCK UP THE GRADE TOWARDS THE FACE AREA WHEN THE MACHINE CAMEOUT OF 1ST GEAR & WENT TO SECOND OR WAS BUMPED OUT OF GEAR & RAN DOWN THE GRADE & INTO A SCOOP W HICH WAS PARKED BELOW IT. THE BUCKET OF SCOOP HIT THE DOOR OF LUBE TRUCK & KNOCKED DOOR INTO OPERATOR CAUSING INJURY.
WAS OPERATING A SHUTTLE CAR. WHEN MADE SWITCH FROM ONE SEAT TO THE OTHER, BUMPED KNEE. WENT SEVERAL WEEKS BUT AFTER CONTINUING PROBLEMS, BECAMEA LOST TIME ON 1-24-96 WITH KNEE SURGERY SCHEDUL ED FOR 2-1-96.
PERSON WAS OPERATING SCOOP IN THE INTAKE AND PARTICLE OF SOMETHING CAME IN UNDER SAFETY GLASSES AND ENTERED RT EYE CAUSING SCRATCH.
THE EE WAS OPERATING A SHUTTLE CAR ON THE 002-0 SECTION WHEN HE STRUCK THE CONTINUOUS MINERS BUMPER THROWING HIM INTO THE CANOPY.
MAN WAS OPERATING THE REMOTE CONTINUOUS MINER. A SHUTTLE CAR CAME IN FOR A LOAD. THE CAR RAN OVER A PIECE OF FIRE CLAY. IT SHOT OUT HITTING HIM ON THE LEFT LEG BELOW THE KNEE.
EMP WAS OPERATING THE SECTION SCOOP WHEN A CABLE CAUGHT HIS HEAD, STRAINING HIS NECK (WHIPLASH) ON THE 001-0 SECTION.

EE WAS SETTING TIMBERS ON THE SUPPLY LOAD WHEN A SCOOP CAME THRU A NEAR BY CURTAIN AND STRUCK AND SUPPLY TRACTOR PULLING A MOTOR M THE WAGON BUMPED HIM. KNOCKING AND HIM TO THE GROUND. DID NOT GIVE TO THE DR UNTIL 8-31-95. FOUND NOTHING BUTOFTEN AND RETURN VISIT ON 9-8-95 THE DR RECOMMOUNDED 2 WKS LIGHT DUTY BECAME LOST TIME.
EE WAS CROSSING OVER A STOPPED BELT. THE BELT STARTED, THROWING HIM TO THE GROUND AND BUISING HIS BACK AND LEG OTHER COMPLICATIONS FROM OUTSIDE SOURCES CAUSING LOST OF TIME JAIL MORITAL LTE CA USED A PROBLEM IN DETERMING IF THEN WAS LOST TIME DUE TO INJURY IT WAS DETERMINED AND LOST TIME ON 12-20-95.
A PIECE OF CURTAIN HAD BECOME ENTANGLED NEAR A BELT ROLLER AS THE EMPLOYEE ATTEMPTED TO REMOE THE SMALL PIECE OF CURTAIN IN THE ROLLER CAUGHT A LOOSE FINGER IN HIS GLOVE & CAUSED HIS ARM TO CO ME IN CONTACT WITH THE CONVEYOR BELT THE FINGER WAS LOOSE IN HIS GLOVE DUE TO A FINGER BEING SURGICALLY REMOVED PREVIOUSLY
INJURED PERSON WAS RIDING INTO MINE OR MINETENDOR. VECHILE THE OPERATOR. SLOWED DOWN TO SHIFT BEARS AND ENGINE STALLED CAUSING VEHICLE TO PICK UP SPEED, PERSON JUMPED OFF AND FELL ON KNEE CAUS ING INJURY. VECHILE WENT ABOUT 2 BLOXKS AND OPERATOR STOPPED VEHICLE. NBO OTHER INJURIES OCCURRED NO VECHILE DAMAGE.
WHILE TRAVELING FROM #1 SLOPE TOWARD #1-A SLOPE, THE EE WAS INJURED WHEN THE NMS 6 TON TRACK LOCOMOTIVE COLLIDED WITH ANOTHER 6- TONTRACK LOCOMOTIVE TRAVELING FROM THE #1-A SLOPE TOWARDS THE # 1 SLOPE. BOTH TRACK LOCOMOTIVES ARRIVED AT OPPOSITE ENDS OF A 45 DEGREE TURN WITH WHICH ALLOWS THE TRACK TO CROSS UNDER E BELT TO ANOTHER ENTRY. LOCOMOTIVES MEET IN THE 45DEGREE.
THE BELT CREW HAD PUT ON (EXTENDED) THE 6T H 54IN BELT LINE THEY RAN THE BELT & THEN STOPPED IT TO SET THE FEEDER AFTER SETTING THE FEEDER THEY WERE GOING TO RUN & ALIGN T HE BELT WHEN EE STAR TED THE BELT THE BELT ANCHOR (JACK PIPE) BENT & THE TAILPIECE PULLED LOOSE HITTING HIM
EE STATED THAT WHILE OPERATING A JOY 21 SHUTTLE CAR ON THE 005 SECTION THE SHUTTLE CAR TRAMMED OVER A ROCK WHICH THREW HIM INTO THE CANOPY INJURING HIS BACK.
MAN WAS ADJUSTING THE FUEL ON DIESEL TRACTOR WHEN HIS FOREARM CAME IN CONTACT WITH THE FAN CAUSING A LACERATION.
MAN WAS WALKING IN THE LAST OPEN CROSS CUT OF A SECTION WHEN A SHUTTLE CAR TURNED A CORNER CAUSING THE CAR CABLE TO CATCH HIS LEG BETWEEN THE CABLE AND THE COAL RIB.
THE BATTERIES WERE DISCHARGED ON HIS SCOOP, REQUIRING IT TO BE PULLED IN. AS A DIESEL SCOOP WAS PULLING HIM THROUGH AN UNDERPASS HIS SCOOP CAUGHT ON THE REAR OF THE SCOOP THAT WAS PULLING HIM, LIFTING HIS SCOOP SLIGHTLY OFF THE MINE FLOOR. WHEN IT CAME LOOSE HIS SCOOP BOUNCED CAUSING HIM TO HIT HIS HEAD ON THE CANOPY.
WAS OPERATING SCOOP SCOOP RAN OVER SOMETHING IN ROADWAY AND CAUSED SCOOP TO HIT HARD JARRING EMPLOYEE CAUSING HIS BACK TO HURT.
MAN WAS CHECKING WATCHING THE BELT FOR BAD SPLICES SOME BELT RAVELINGS STRINGS HAD WRAPPED AROUND A ROLLER AND WAS HANGING DOWN. HE PLACE HIS HAND NEAR THE ROLLER AND WHEN THE ROLLER TURNED ON E OF THE RAVELING CAUGHT HIS FINGER AND PULLED IT INTO THE ROLLER CAUSING A LACERATION.
THE EMPLOYEE WAS OPERATING A MOBILE BRIDGE CARRIER WHEN HE LET THE CONVEYOR BOOM DROP DOWN CATCHING HIS LEFT RING FINGER CAUSING A LACERATION ON 001-0.
THE EMPLOYEE WAS CRAWLING UNDER THE BRIDGE CONVEYOR WHEN THE OPERATOR LOWERED THE BRIDGE CATCHING HIS SHOULDER.
EE STATED THAT AFTER HE GOT A LOAD FO ROCK, HE TURNE DAROUND ON CAR AND TWISTED LEFT KNEE.
MAN WAS TRAMMING A SUPPLY JEEP THROUGH A CURTAIN WHEN HE CAUGHT HIS ARM BETWEEN THE JEEP AND A SUPPLY TRAILER.
BRATTICE MAN WAS CONSTRUCTING A BRATTICE IN THE THIRD OPEN X-CUT-INTAKE SIDE. THE CR HAD BEEN TRAVELING THROUGH THIS X-CUT BUT HAD TO REPAIR CABLE/BRATTICE MAN STARTED BRATTICE AND HAD IT 3/4 STACKED WHEN THE CABLE WAS REPAIRED AND THE CAR RESUMED OPERATIONS. CAR WENT THRU THE CURTAIN AND HIT BRATTICE - TRAPPING BRATTICE AGAINST SUPPLY CAR. BOTH LEGS DAMAGED.
INJURED STATED HE WAS SITTING ON BACK OF A 3 WHEELER AND WHHEN IT CAME OUT OF CROSSCT HIS HEAD HIT THE ROOF CAUSING HIM TO JAM HIS NECK.
INJURED STATED THAT HIS LEFT FOOT WAS HANGING OUT OF MANTRIP AND WAS CAUGHT BETWEEN TIMBER AND SIDE OF MANTRIP WHEN MANTRIP MADE TURN.
PERSON WAS OPERATING LOCOMOTIVE DELIVERING SUPPLIES TO SECTION PERSON LOST CONTROL OF MOTOR & IT BEGAN TO SLIDE HE JUMPED OFF MOTOR & STRUCK HIS HEAD AGAINST STEEL COAL POST HOLDING A X-BAR UP
INJURED STATED THAT HE WAS OPERATING A SCOOP WHEN HE RAN OVER A LUMP OF COAL CAUSING HIM TO JAM HIS HEAD INTO ROOF.
MAN WAS RIDING IN A DIESEL MANTRIP THAT WAS BEING BROUGHT TO BOTTOM A SCOOP OPERATOR HAD PARKED HIS SCOOP (NEAR A CURTAIN) TO HELP A SUPPLY MAN WITH HIS EMPTIES THE TRIP CAME THROUGH THE CURTA IN & HIT THE SCOOP A SCSR BOX HIT EE IN THE BACK OF THE HEAD
HE WAS BRINGING A DIESEL MANTRIP TO BOTTOM FOR DAY SHIFT. A SCOOP OPERATOR HAD PARKED NEAR A CURTAIN TO HELP A SUPPLY MAN WITH THE EMPTIES. THE INJURED AND A PASSENGER CAME THROUGH THE CUR TAIN HITTING THE SCOOP. THIS THREW INJURED INTO THE STEERING WHEEL.
BILL STATED HE WAS WORKING ON THE CHUTE ON THE BELT HEADER. HE WAS STANDING ON THE BELT ON THE BACK SIDE OF THE CHUTE WHEN HIS WORKING BUDDY STARTED THE BELT. HE FELL TO THE FELT AND WENT UN DER THE CHUTE BEFORE GETTING OFF. SAW CHIROPRACTOR FOR FIRST TIME ON 10-27-94.
EE WAS TURNING AROUND ON A SHUTTLE CAR AFTER GETTING A LOAD FROM LOADER IN #6 ENTRY AND TWISTED HIS RIGHT KNEE/ HIS FOOT CAUGHT ON THE CAR SEAT. HE DIDN'Y MISS ANY DAYS UNTIL 10-24-94 WHNE HE HAD HIS KNEE OPERATED ON.
PERSON WAS LIFTING UP ON COUPLER ON FLAT CAR WITH RT HAND. PERSON HAD HIS LEFT HAND ON COUPLER ON MOTOR, WHEN COUPLERS CAME TOGETHER HIS LEFT INDEX FINGER WAS CAUGHT BETWEEN THE COUPLERS.
INJURED HAD SPOTTED AN EMPTY TRAILER. HE WAS BACKING #54 TRACTOR UP TO HOOK IT TO HIS DUSTER WHEN HE HIT HIS HEAD ON A PIN BOARD. TOP WAS LOW. HE STRAINED HIS NECK.
MINER HELPER WAS MOVING CABLE AND WATER LINE OUT OF HAULAGE ROAD. THE LEFT DRIVE SHUTTLE CAR OPERATOR DID NOT SEE HIM AND DROVE THE SHUTTLE CAR UP ON THE MINER HELPER'S HAND.
INJURED WAS STAGGERING OUT A SHUTTLE CAR. WHILE GETTING A LOAD OF COAL A LUMP POF COAL BOUNCED OFF THE CAR FRAME AND STRUCK HIS KNEE. HE WAS HAVING TROUBLE WITH HIS KNEE DUE TO LOCKING US AN D SWELLING BEFORE THIS INCIDENT. HE WORKED UNTIL JUNE THE 7TH WHICH WAS THE FIRST DAY HE MISSED AND WENT TO THE DR WHICH SAID HE NEEDE DSURGERY.
WHILE CHECKING FOR A HYDRAULIC LEAK ON THE ROAD GRADER THE EMPLOYEE HAD HIS HAND ON THE HOSE WHEN THE GRADER OPERA TOR ENGATGED THE HYDRAULICS THE LEAKING HOSE SHOT HYDRAULIC PRESSURE (FLUID) INTO THE EMPLOYEES HAND WHICH DEVELOPED INTO INFECTION EMPLOYEE 1ST DAY OFF WAS 06-01-94
EMPLOYEE STATES THAT THROTTLE STUCK ON END-LOADER HE WAS OPERATING CAUSING THE MACHINE TO SRIKE A METAL BEAM FORCING HIS BODY INTO THE WINDSHIELD CAUSING BACK, NECK AND SHOULDER PAIN.
INJURED STATED TO THE BEST OF HIS KNOWLEDGE HE WAS CLEANING ON THE E BELT . HE DECIDED TO CROSS THE BELT. AFTER THROWING HIS SHOVEL TO THE OTHER SIDE, HE STARTED TO CROSS THE BELT WHEN THE BEL T STARTED, DRAGGING HIM UNDER THE HEAD FRAME OF THE FEEDER. HE ROLLED OFF THE BELT AND WAS CAUGHT BETWEEN THE TOP AND BOTTOM BELT FOR A PERIOD OF TIME BEFORE BEING ABLE TO GET OUT.
