Employee, parked s/c on grade at 16-Block #3 Entry Spad #3868. S/c slid approx. 3' pinning oper. between s/c & rib, employee had set park brake and got off machine to move roof bolter cable.
Employee was scooping #4 Hdg. and curtain fell down. Reached up to get curtain and finger got between canopy and roof. Cutting off end of finger.
Employee operating diesel scoop. Ran over larger rock scoop raised up quickly, employee hit heat, knocked off hard hat and then head hit a steel strap (edge), cut top of head requiring stitches.

EE was pushing on man trip (2-person ride) in 6 entry Blk 24 5G section. On hill put fingers between bar guard and battery case, battery slid back mashing 2 fingers.
Front wheel on 2 man rubber tired mantrip caught rail in track entry and steering wheel whipped and cut operator's finger.
Employee riding in mantrip. Left foot was extended outside of compartment. Mantrip close to rib, mashed left foot between rib & mantrip. Fractured big toe on left foot.
Backing up mantrip, tire hit rib and steering wheel spun around. Knob on steering wheel hit index finger on right hand.
Maintenance Foreman was using end loader forks to apply pressure to a bent drag link, trying to straighten it. Smashed his left thumb.
Employee was running Lo-Trac scoop. He hit his head on fly board, snapping neck back.
EE riding in mantrip, right foot was extended outside compartment. Mantrip hit rib opposite machine operator. Right foot caught between rib and side of mantrip. Smashed metatarsal and cut boot. The right 3rd and 4th toes caught and cut at 1st joint.
EE WAS USING SCOOP TO MOVE A HEAD DRIVE, PUSHING AGAINST 2 HEADER BOARDS. THE 2 BOARDS FLIPPED OUT AND SMASHED HIS FINGER BETWEEN THE BOARDS AND THE FRAME OF THE HEAD DRIVE. (AMPUTATION: 1/2 MIDDLE FINGER ON LEFT HAND.)
SCOOP OPERATOR WAS UNLOADING A PIECE OF METAL FROM SCOOP BUCKET. HE STEPPED BETWEEN THE BUCKET & A MINE POST. THE SCOOP DRIFTED FORWARD & SMASHED HIS LEG. HE WORKED UNTIL 1/11/06. HIS LEG SWELLED UP & THE DR. TOLD HIM TO STAY OFF OF IT & KEEP IT ELEVATED.
EE running shuttlecar, miner cable was lying close to car as it was operating. As car was being trammed operator reached left arm out to move cable. Left arm wedged between rib and bottom of canopy causing top of left arm to be busted open. Required 30 to 40 stitches.
MECHANIC WAS STANDING BETWEEN SCOOP BUCKET AND MINER RIPPER HEAD. SCOOP DRIFTED 2 FEET AND SMASHED HIS LEG. #7 ENTRY L.O.C. 2 NE E PANEL.
While riding in rail mantrip, individual struck forehead on belt guard where the rail goes under #3 belt line.
SHUTTLE CAR OPERATOR GOT OFF CAR AND WALKED AROUND IN FRONT OF MACHINE. IT DRIFTED OFF, RUNNING UP ON OPERATOR'S FOOT.
HE WAS RIDING ON THE BACK OF THE 3 WHEELER ON KNEES, TOES HANGING OVER EDGE. THEY STARTED BACKING UP TO THE ROOF BOLTER TO UNLOAD AND SMASHED FOOT BETWEEN ROOFBOLTER & 3 WHEELER.
CHANGING TANDEM ROLLER ON BELT DRIVE, HE WAS PULLING BULL GEAR TOWARD HIMSELF. THE OTHER END OF ROLLER FELL OFF THE FRAME AND HIS END WENT UP, SMASHING HIS FINGER BETWEEN BULL GEAR & FRAME.
EE WAS SITTING ON 5 TAIL CHECKING BELT SPLICES. TEH CONVEYOR BELT CAUGHT HIM TOOL BELT AND DRAGGED HIM UP ONTO #5 BELT CONVEYOR. HE WAS THEN FORCED UNDER 5A HEAD DRIVE ROLLER AND THEN FORCED H IM UNDER A WOODEN CROSSOVER OUTNBY 5A HEAD ROLLER. HE THEN ROLLED OFF 5 BELT.
MANTRIP OPERATOR WAS RUNNING RAIL BUS OUTSIDE AT END OF SHIFT. HE DUCKED HIS HEAD TO MISS WATER LINE WHICH WAS HUNG ON ROOF. AFTER CROSSING UNDER WATER LINE HE RAISED HIS HEAD BACK UP AND CAUG HT THE ROOF. A ROOF PLATE CUT HIS FOREHEAD ABOVELEFT EYE.
A FLAT CAR WAS ON THE END OF THE SECTION TRACK AND THE CREW CAME DOWN AND GOT ON THE MANTRIP TO LEAVE THE SECTION WHEN THEY GOT TO THE CURVE THEY SLOWED DOWN TO GO AROUND THE CUREVE WHEN THEY SAW THE FLATCAR COMING TOWARD THEM & IT HIT THE MANTRIP.
THE EMPLOYEE WAS WORKING AROUND TAIL PIECE AND FELL INTO THE BELT CAUSING A BRUISING OF HIS LEG.
EE WAS DRIVING RUBBER TIRED MANTRIP, THE FRONT TIRES HIT A RUT AND THE STEERING WHEEL FLEW OUT OF HIS HANDS. THE KNOB ON THE STEERING WHEEL HIT THE BACK OF HIS HAND.
