Employee was driving up out of pit and the sun blinded him and he run his water pumping truck up a bank and laid it over on its side. He worked the rest of shift and the next day and then took 2 days off.
Employee was standing out and in front of a 345 Trac hoe giving directions to the operator helping him guide the track back on that had jumped off the bottom rollers when a metal splinter came off and entered his arm causing a cut muscle.
Employee was running a loader and was thrown from his seat into the upper part of cab hitting his head on a radio causing a cut requiring 4 stitches.
Employee was operating another's 2011 pickup alone. Truck required servicing to clear mud from transmission linkage preventing transmission from being put into park and normal operation. Employee stopped truck, crawled underneath to service linkage and the transmission engaged in reverse fracturing employees left arm while he attempted to get out of the way.
Employee was operating a 777 Cat rock truck and was being loaded by a 992 cat loader. He claims a rock slid over and hit the inside of the bed and jarred him. Went to Twin City Chiropractic center and they gave him 3 days off.
EE was backing his Liebherr Rock Truck under the Liebherr Loading Shovel and made contact with the bottom of the bucket. EE strained his neck
Injury was coming down a return ramp into the pit to get loaded again when he stated his brakes failed. He yelled for the loader man to move out of the way He ran his 777B end dump into the highwall. He received a broken left leg.
EE was running a 992 Cat front-end loader and was loading rock trucks. He was backing away from a truck with his boom in the air, he had not noticed that the keeper had come out of pin housing, which let the boom pin come out. The boom dropped and it shook the operator. Took two days off for soreness.
Was backing up to the edge of end dump fill and ran his right side tires off edge, causing end dump to roll over onto its top. His metal lunch box flew and hit him in the back of the head, causing a gash in head, requiring stiching.
Employee was traveling up a grade when he ran out of fuel stalling the engine. Without maxi brakes that lock up when depletion of air the truck could not start, it started to coast backwards and struck a birm and rolled onto its side. Employee was wearing his seat belt.
EMPLOYEE WAS BACKING THE WATER TRUCK DOWN A PIT RAMP IN LOW RANGE WITH THE ENGINE RETARDER ON AND APPLIED THE BRAKES. THE PEDAL FELT SOLID THE FIRST TIME BUT THE SECOND TIME HE APPLIED THE BRAKES AN AIR LINE POPPED OFF AND HE LOST HIS BRAKES AND AIR. TRAVELING AT A HIGHER RATE OF SPEED IN REVERSE, THE BACK DRIVER'S SIDE DUALS CLIMBED UP THE BANK AND FLIPPED OVER.
EMPLOYEE WAS TRAVELING DOWN A SLIGHT GRADE WITH A TURN TO THE RIGHT AT THE BOTTOM WHEN HIS FRONT TIRE SLID AND THE BED OF THE TRUCK TO JACK KNIFE SIDEWAYS CAUSING THE FRONT END TO STAND UP ON ITS NOSE THROWING HIM FOREWARD IN THE CAB. SEAT BELT WAS WORN.
EMPLOYEE WAS TURNING THE WATER TRUCK AROUND AT THE END OF THE ROAD WHEN A COAL TRUCK CAME UP ON HIM WHILE EMPLOYEE WAS SIDEWAYS IN THE ROAD SO EMPLOYEE COASTED BACK TO GIVE THE TRUCK DRIVER MORE ROOM AND HIS BACK TIRES FELL OFF THE ROAD AND INTO A SUMP HOLE AREA AND ROLLED OVER ON IT'S SIDE.
Employee was riding in the back seat of a company truck when the driver struck a birm on the right side throwing him into the air inside the cab. He was bounced off of the roof and the landed sideways on his lunch box. ...Reason for tardiness --- Unaware of e-mail problems with computor...
EE WAS LOWERING AN OBJECT TO THE GROUND WITH HIS BOOM AND WHEN SLACK WAS IN THE CABLES EE GRABBED THE CABLE AND RE-POSITIONED THE BLOCK AND THE OBJECT SLIPPED OFF AN UNSEEN OBJECT ON THE GROUND CAUSING HIS FINGER TO BE PULLED INTO THE PULLEY.
EE CAME TO A STOP ON AN INCLINE AT A ROAD CROSSING. WHEN HE PROCEDED FORWARD, EE STALLED THE TRUCK. WHILE ATTEMPTING TO RESTART THE TRUCK, EE DID NOT APPLY THE EMERGENCY BRAKE & EXPIRED AIR SU PPLY IN THE TANKS. THE TRUCK COASTED BACKWARDS & OVERTURNED. THE CB MICROPHONE HIT EE IN THE MOUTH CAUSING STITCHES. THERE WAS NO TIME LOST.
EE PARKED THE FUEL TRUCK 50 FT AWAY FROM THE ROCK TRUCK DURING LUNCH BREAK. AFTER BREAK, TRUCK OPERATOR BACK UP NOT KNOWING THE FUEL TRUCK WAS THERE. THE ROCK TRUCK SLAMMED INTO THE BACK OF TH E FUEL TRUCK RIPPING THE BUMPER OFF. JUST AT THAT TIME. THE ROCK TRUCK TURNED HIS WHEELS AND PULLED AWAY.
