STANDING BEHIND THE HOPPER SHOVELING TAIL WHEEL,PIT TRUCK DUMPING IN HOPPER,SOME COAL CAME OVER THE SIDE HITTING ON TOP OF HEAD.
EMPLOYEE WAS DR3VING WATER TRUCK, GOING DOWN INCLINE TO PIT. TRUCK STARTED GOING TOO FAST. INSTEAD OF HITTING BRAKE, HE TRIED TO SWITCH TO LOWER GEAR, COULDN'T GET IT BACK INTO GEAR. BY THEN H E WAS GOING TOO3FAST FOR BRAKES TO HOLD TRUCK, DROVE OVER SAFETY BERM AND ROLLED OVER.
