I,NAME'S DUTIES CONSISTED OF COLLECTING COAL SAMPLES FROM VARIOUS LOCATIONS ON K.C.C.'S ALLOY LOADOUT. ON THE DAY OF THE ACCIDENT, I,NAME DROVE HIS TRUCK OFF OF A COAL STOCK PILE & FALLING APP ROX. 18', RESULTING IN INJURY TO HIM.
