Showing all 22EE parked mantrip, got out, was getting chock block out of rear compartment. Truck started to roll down hill, EE tried to get back in truck and fell dwon or was knocked down by rear compartment of truck. This resulted in 2 broken ribs and broken bone ni right foot (metatarsus).
Bullgang crew was using skid steer to stand up door frame for equipment doors. Frame was chained to forks. Employee was moving cable out of the way when chain came unhooked and frame fell on top of his right foot. Fracture to small bone on top of foot.

The EE was moving manlift from inside the manbasket in the surface yard. The platform was tilted down and wouldn't return to a level position, so he placed the manbasket on a shield and applied downward pressure with the machine. The manbasket slid off the shield, hitting the ground. The EE fractured his right foot above his ankle.
Employee was operatng Getman tractor, hauling pod duster into the mine. Lost control on wet road and jack knifed at bottom of slope. Canopy caught on tunnel liner and crushed cab. Employee bruised his neck and back from the impact.
The EE had his hand outside the canopy and caught his hand between the canopy and rib. Laceration on left index finger, 20 stitches to close the wound.

EMPLOYEE REPORTED TO HIS FOREMAN AT THE BATH HOUSE AFTER HIS SHIFT, WHILE HE WAS OPERATING THE SHUTTLE CAR ON HIS SHIFT HE HIT A HOLE IN THE ROADWAY & HIS HEAD STRUCK THE ROOF OF THE CAB. NECK STRAIN.
EE WAS ASSISTING IN SETTING STEEL ARCHES AT THE RETURN AIR SHAFT. HE WAS GUIDING THE 3 1/2 SCOOP INTO THE LEG ON THE ARCH TO INSTALL THE SPREADER PIPE AND BOLT. AS THE LEG WAS MOVED IN, HE PLACED HIS R. MIDDLE FINGER BETWEEN THE PIPE AND ARCH, RESULTING IN THE TIP OF HIS FINGER BEING AMPUTATED. EE WAS TRANSPORTED TO THE HOSPITAL AND THE FINGERTIP WAS REATTACHED.
EE WAS UNHOOKING SKID FROM WAGNER SCOOP, WHEN THE RAM CAME FORWARD AND PINCHED LITTLE FINGER ON LEFT HAND WHILE HE WAS UNHOOKING CHAIN. LACERATION WHICH REQUIRED 7 STICHES TO CLOSE WOUND.
EE WAS BACKING MATERIAL HAULER INTO X-CUT WITH POD DUSTER HOOKED ON TO FILL DUST TANK. REACHED OUT OF CAB BETWEEN CASE AND BODY OF GETMAN AND GRABBED THE STOP BLOCK. HIS FINGER WAS CAUGHT IN THE PINCH POINT, COMPOUND FRACTURE RIGHT HAND RING FINGER.
EE WAS OPERATING DIESEL SCOOP, WHEN HE HIT A BUMP TO THE ROAD. CAUSING HIS HEAD TO HIT THE CAB RESULTING IN A NECK STRAIGHT.
EE REACHED UNDER THE BELT TO GREASE THE CENTER ROLLER ON THE TROUGHING ROLLER ASSEMBLY. HIS SHIRT GOT CAUGHT BETWEEN UPPER ROLLER AND BELT AND PULLED HIS ARM BETWEEN ROLLER AND BELT.
OPERATING 950 LOADER COMING DOWN FROM ELK CREEK MINE. APPLIED BRAKES AND THE ENGINE DIED CAUSING LOSS OF CONTROL ON STEEP GRAD, WENT OVER BERM, DOWN HILL TO CREEK BOTTOM. BROKE RIBS AND VERTEB RAE ON IMPACT. CLIMBED OUT OF CREEK BOTTOM, WALKED DOWN ROAD TO GET HELP.
HE WAS DRAGGING VENTILATION TUBING, THE SHUTTLE CAR DRIVER TOLD HIM TO MOVE OVER, THE SHUTTLE CAR HIT THE VENT. TUBING AND IT HIT EE ON THE SIDE OF THE LEFT KNEE.
EE WAS STANDING IN THE BUCKET OF SCOOP HANGING 7200 CABLE WHEN HE HIT THE ROOF WITH THE TOP OF HIS HARD HAT HURTING HIS NECK. NO MEDICAL REPORTS HAVE BEEN RECEIVED AND A NOTICE OF CONTEST HAS BEEN FILED.
OPERATING SHUTTLE CAR HIT BUMP IN ROADWAY, BOUNCED UP HITTING HEAD ON CAB OF SHUTTLE CAR. THERAPY SESSIONS FOR NECK PAIN.
HE WAS OPERATING LOADER WITH DUSTING POD TAKING IT BACK TO RELOAD. THOUGHT HE HEARD SOMETHING DRAGGING TURNED TO LOOK, WENT INTO DITCH AND KNOCKED THE RIB DOWN ON HIM HITTING LEFT SIDE OF HIS FACE.
