SHUTTLE CAR OPERATOR WAS PULLING HIS SHUTTLE CARCABLE ACROSS THE 014 MMU SECTION BELT WHILE IT WAS OFF TO ANCHOR IT IN ANOTHER LOCATION. AS HE CROSSED THE BELT IT SUDDENLY STARTED AND HIS RIGH T ANTERIOR THIGH WAS ABRAIDED BY A TOP ROLLER ASHE GOT OFF THE BELT. HE HAS DEVELOPED AN INFECTION, SAW HIS PHYSICIAN ON 8/27/01 AND STARTED OFFWORK ON THE SAME DATE.

THE EE JAMMED HIS NECK WHEN A SCOOP HE WAS OPERATING SLIPPED OFF BLOCKING AND BOUNCED HIS HEAD AGAINST THE ROOF OF THE SCOOP CANOPY.
WHILE HAULING SUPPLIES ON A SUPPLY CAR, THE CAR WRECKED. WHEN HE ATTEMPTED TO PULL THE CAR BACK ONTO THE MINE TRACK, A CHAIN BROKE AND STRUCK HIS RIGHT ELBOW. (CHIPPED BONE).
EMPLOYEE WAS OPERATING MANTRIP. AS HE STARTED UNDER THE BELT ON E-MAINS TRACK- THE TRIP STRUCK THE GUARD AND DISLODGED IT. HE WAS CAUGHT BY THE GUARD AND TWISTED AROUND IN THE OPERATIONS COMPA RTMENT. HE DEVELOPED LOWER BACK PAIN. NO DAYS WERE LOST MEDICAL TREATMENT ONLY.
WHILE OPERATING A 10 TON LOCOMOTIVE ON THE 6-MINER TRACK, THE INJURED APPLIED HIS BRAKES & LOST TRACTION DUE TO WET RAILS. THE LOCOMOTIVE THEN RAN INTO AN UNOCCUPIED MANTRIP PARKED AT THE END OF THE TRACK. DID NOT BEGIN MISSING WORK UNTIL 10/2/00.
EE WAS TRAVELING THROUGH AN INTERSECTION ON THE D-6 PANEL. A SHUTTLE CAR CABLE WAS HUNG FROM THE ROOF BOLTS ACROSS THE INTERSECTION. AS HE WALKED ACROSS THE INTERSECTION THE SHUTTLE CAR WAS ST ARTED AND MOVED. THE CABLE CAME DOWN & STRUCK HIM ACROSS THE FACE AT EYE LEVEL. THIS RESULTED IN A FRACTURE OF HIS NASAL BONE. HE MISSED NO WORKDAYS. HIS NASAL FRACTURE WAS SPLINTED-MED TREAT
WHILE BACKING UP THE SCOOP OUT OF X-CUT S-6, THE CANOPY CAUGHT THE END OF A CABLE TRUSS, WHICH FLEXED AND FLIPPED BACK INTO THE DECK STRIKING THE OPERATOR OVER THE LEFT EYE.
RIDING IN MANTRIP, MANTRIP DERAILED CAUSING HIM TO HIT HIS ELBOW.
EMPLOYEE WAS STEPPING OFF THE CONVEYOR BELT AFTER HE AND ANOTHER EMPLOYEE HAD COMPLETED WORK ON THE BELT SKIRT AT THE DISCHARGE POINT. THE BELT WAS SET IN MOTION CAUSING THE EMPLOYEE TO FALL O FF BELT. THIS RESULTED IN AN INJURY TO EMPLOYEE'S LEFT KNEE.
EE STATED HE WAS REPAIRING SWITCH, A MAN BUS CAME BY AND A CROSS BAR CAME DOWN ON HAND.
A SCOOP WAS BEING USED TO REMOVE A 1000FT STACK OF CONVEYOR BLET 16"HIGH OVER TO THE BELT TAKEUPON A-1 PANEL. THE BELT BECAME CAUGHT AGAINST THERIB. EE WENT BETWEEN THE SCOOP & RIB TO REHOOK THE CHAIN. HIS RIGHT FOOT SLIPPED OFF OF THE BELT & BECAME STUCK BETWEEN THE BELT & RIB. HE TOLDTHE SCOOP OP. TO SKEW THE BELT AWAY FROM THE RIBTHE OP SKEWED THE SCOOP BILL AWAY FROM THE RIB
INDIVIDUAL WAS RIDING ON THE IN BY END OF MANTRIP WHERE IT JUMPED, AT THE #3 SPUR ON 2ND TRACK. HIS LEGS WERE CROSSED AT THE TIME AND WHEN THE MANTRIP FELL FROM THE RAIL, IT BOUNCED HIM UP IN THE AIR CAUSING HIS RIGHT HAND TO STRIKE THE TOPEDGE OF THE SIDE PANEL.
THE INDIVIDUAL WAS ASSISTING A WORKMAN IN INSTALLING A 48" CRADLE ROLLER ON THE BELT ROPES OF THE MAIN WEST #2 BELT, WHEN HE FELT PAIN IN HIS RT SHOULDER JOINT. THE INJURED DID NOT BEGIN MISSI NG WORK UNTIL 1/20/99.
WHILE SITTING DOWN IN THE MANTRIP, THE INJURED STRUCK HSI RIGHT ELBOW ON THE BACK OF TH METAL SEAT.
THE INJURED HAD LOCKED THE BRAKES ON THE MOTOR, DISMOUNTED AND WENT TO UNCOUPLE THE MOTOR FROM THE SUPPLY CARS WHEN HE SLIPPED. WHEN HE CAUGHT HIMSELF FROM THE SLIP HE HAD PLACED HIS LEFT FOOT AGAINST THE RT INBY WHEEL OF THE TRANSPORT CAR AT THAT TIME THE OUTBY MOTORMAN THOUGHT THAT HE WAS FLAGGING HIM TO DROP THE CARS DOWN, WHICH IN TURN RAN OVER HIS FOOT
THE INDIVIDUAL HAD HAND ON BELT SCRAPPER FRAME WHEN BELT SPIKE PASSED THROUGH IT CAUSING IT TO JUMP. WHICH RESULTED IN A LACERATION TO HIS FINGER HE WAS WEARING GLOVES AT THE TIME OF ACCIDENT
THE INJ WAS STANDING ALONG SIDE THE A MAINS NO.3BELT CHECKING SPLICES IN THE BELT WHEN THE JACK'PIPES USED TO ANCHOR THE TAILPIECE LET LOOSE AND ALLOWED THE TAILPIECE TO KICK SIDEWAYS STRIKING THE INJ ON THE RIGHT KNEE RESULTING IN A LACER ATION WHICH REQUIRED 6 STITCHES.
THE INJURED WAS FOUND CAUGHT BETWEEN THE LOLO DOLLY AND THE B MAINS BELT, THE BELT WAS RUNNING AT THAT TIME. THE INJURED WAS CONSCIENCE. THE RESULTING INJURIES WERE A FRACTURED LEFT ANKLE, 6 F RACTURES OF THE PELVIS AND A DISLOCATED RIGHT SHOULDER. ACCIDENT IS CURRENTLY UNDER INVESTIGATION PENDING INTERVIEW WITH THE INJURED.
THE INJURED WAS OPERATING THE NO. 4 (OUTBY) BRIDGE CARRIER. THE 4 UNIT SYSTEM WAS BACKED OUT THE BELT ENTRY. WHEN THE TRAMMING STOPPED THE EE STEPPED OUT OF THE OPERATORS' COMPARTMENT TO LOOK TO SEE HOW MUCH TRAVEL SPACE HE HAD BEHIND HIM WHEN THE BRIDGE IN FRONT OF HIM TRAMMED BACK AND CAUSED HIS BRIDGE TO KICK SIDEWAYS, PINCHING HIS RIGHT LOWER LEG BETWEEN THE LO-LO STRUCTURE AND
INJURED HAD LEFT MIDDLE FINGER MASHED BY THE BATTERIES ON THE OUTBY END OF #36 MANTRIP WHEN IT STRUCK #3 MANTRIP.
THE INDIVIDUAL WAS A PSSENGER ON THE #39 MANTRIPWHICH COLLIDED WITH #17 MANTRIP WHILE TRAVELING TO THE 2NE SECTION.
THE INDIVIDUAL WAS A PASSENGER ON THE #39 MANTRIP WHICH COLLIDED WITH #17 MANTRIP WHILE TRAVELING TO THE 2 WE SECTION.
THE INDIVIDUAL WAS A PASSENGER ON THE #39 MANTRIP WHICH COLLIDED WITH A #17 MANTRIP WHILE TRAVELING TO THE 2NE SECTION.
WHILE USING A TRACK MOTOR TO PUL A BELT DRIVE INTO THE MINE WITH A 1/2 INCH CHAIN (THE BELT DRIVE WAS MODIFIED TO SLIDE ON THE TRACK RAIL), THE CHAIN BROKE STRIKING THE INJURED ON THE LEFT SID E OF HIS FACE AND HEAD.
EE WAS TRAMMING A 3 WHEELER FROM END OF TRACK TO THE SECTION WHEN THE STEERING WHEEL SPUN AROUND AND STRUCK EE'S RT ARM, CAUSING A BRUISE.
WHILE USING A SHOVEL TO CLEAN A BELT PAN LINE INTHE RAW COAL TUNNEL AT THE PREP PLANT, THE INJURED'S GLOVE ON HIS LEFT HAND CAUGHT ON THE BELT AND PULLED HIS ARM BETWEEN THE BELT AND A BELT RO LLER RESULTING IN BRUISE AND SWELLING OF HIS LEFT FOREARM.
ON HAUL ROAD.
WHEN TRAMMING THE OFF STANDARD SHUTTLE CAR UP THE CONTINUOUS MINER THE SCRUBBER DISCHARGE BLEW DEBRI INTO THE INJURES LEFT EYE. THREE DAYS LATER HIS EYE BECAME INFECTED WHICH RESULTEDIN THIS L OST TIME ACCIDENT. THE INJURED DID NOT BEGIN MISSING WORK UNTIL 8-4-96.
THE INJURED WAS ACTIVING AS ASSISTANT TO THE SUPPLY LOCOMOTIVE OPERATOR, WHEN SWITCHING SUPPLY CARS IN THE 1 NORTHEAST SECTION TRACK, HIS SHIRT COLLAR GOT CAUGHT ON A ROOF BOLT PLATE PULLING H IM OFF OF THE SUPPLY CAR HE WAS RIDING ON.
HANDLE ON SUPPLY CAR WOULD NOT UNCOUPLE CARS SO EE POSITIONED HIMSELF BETWEEN CARS TO DISCONNECT THEM. MOTORMAN MOVED CARS SLIGHTLY CAUSING WHEEL ON A SUPPLY CAR TO RUN UP ON FOOT OF INJURED.
EE ALLEDGED THAT HIS NECK AND BACK WAS STRAINED, WHEN HE RAN OVER LARGE ROCK IN ROADWAY WITH SCOOP, AND IT THREW HIM AGAINST CANOPY.
EMP ALLEGED THAT HE STRAINED HIS NECK WHEN THE SHUTTLE CAR HE WAS OPERATING HIT A HOLE IN THE ROADWAY AND BOUNCED HIM UP AGAINST THE CANOPY.
THE INJURED WAS OPERATING THE OFF STANDARD SHUTTLE CAR WHEN A ROCK APPROX THE SIZE OF A SOFTBALLWAS DISCHARGED FROM THE MINER CONVEYOR CHAIN DURING LOADING OF THE SHUTTLE CAR. THE ROCK STRU CK THE INJURED ON THE RIGHT SIDE OF HIS FACE. THE INJURED ID NOT LOOSE CONSCIOUSNESS
EMPLOYEE WAS TRAMMING SHUTTLE CAR ACROSS SECTION HE HIT A HOLE IN THE BOTTOM IT THREW HIS HEAD UP AGAINST CANOPY & STRAINED HIS NECK
INJURED WAS BRINGING COAL FEEDER UP SLOPE WHEN HOIST ROPE JERKED AND HIT HIS FOOT.
RAIL MANTRIP CAME OFF TRACK AND JARRED HIM.

WHILE EXITING MINE IN A N OPEN MANTRIP WITH THE CREW A ROCK FELL FORM THEH ROOF STRIKING THE INJURED ON THE LEFT FORARM, CAUISNG ABRASIONS AND BRUISES. AND STRIKING HIS LEFT TESTICLE, CAUISNG SORENESS & SOME SWLLING THE ROCK'S SIZE WAS 8"X10"X1 1/2". THIS OCCURRED 3 BLKS INBY 3 EAST /V-MAINS SWITCH WHICH IS WELL SUPPORTED.

INJURED WAS GETTING CANS OF HYDRAULIC OIL OUT OF SCOOP BUCKET WHEN ANOTHER SCOOP STRUCK THE SCOOP FRO MBEHIND SHOVING THE SCOOP FORWARD A GAINST HIS FOOT
INJURED WAS RIDING TRACK LOCOMOTIVE TO SECTION WHEN POLE CAME OFF TROLLEY WIRE AND STRUCK MIEN ROOF WHICH LOOSENED ROCK WHICH FELL ON HIM.
INJURED WAS CROSSING OVER BELTLINE WHEN THE BELT STARTED THROWING HIM AGAINST MINE ROOF AND FLOOR.
INJURED WAS LOADING SUPPLIES ONTO A SUPPLY CAR WHEN THE CAR WAS STRUCK BY ANOTHER CAR CAUSING HIM TO GET HIT BY FLYING MATERIAL.
