EMPLOYEE WAS STEPPING OVER SCOOP BUCKET WHEN THE SCOOP OPERATOR INADVERTENTLY MOVED HIS BUCKET CATCHING THE EMPLOYEE'S LEG BETWEEN HIS SCOOP AND A SCOOP THAT WAS PARKED ALONGSIDE HIM.
THE EMPLOYEE WAS OPERATING A LONG-AIRDOX PERSONNEL CARRIER IN THE MAIN INTAKE TRAVELWAY. I,NAME HIT AN UNEVEN AREA IN THE MINE FLOOR CAUSING HIM TO BE BOUNCED IN THE AIR AND UPON LANDING BACK IN THE SEAT, HE NOTICED PAIN IN HIS LEG AND BACK
EMPL HAS OPERATED JOY 21-SC SHUTTLE CAR FOR PAST 2 YEARS IN COAL HEIGHT RANGING FROM 40" TO 52" WITH HARD LIMESTONE BOTTOM THESE CONDITIONS BOUNCING & STRAIN CONTRIBUTED TO INJURY.
THE EMPLOYEE WAS IN THE PROCESS OF REMOVING OVER-SIZE ROCK FROM THE COAL FEEDER (002 SECTION). INJURED PARKED SHUTTLE CAR AT FEEDER, SET A BLOCK UNDER WHEEL, AND WAS POSITIONED BETWEEN IT AND THE COAL FEEDER. THE SHUTTLE CAR DRIFTED OVER THE BLOCK ADN PINNED INJURED'S LEFT FOOT AGAINST THE FEEDER CAUSING THE CRUSHING INJURY.
THE EMPLOYEE WAS OPERATING A JOY 21-SC SHUTTLE CAR AND PERFORMING ROUTINE FACE WORK DURING THE SHIFT. AT SOME POINT DURING THE SHIFT A FOREIGN PARTICLE ENTERED THE EYE CAUSING IRRITATION AND IRRIGATION REMOVAL WAS NECESSARY.

EMP WAS IN PROCESS OF CLEANING UP ROCK WITH A MODEL 482 S&S SCOOP TRACTOR. I NAME WAS HAULING LOAD OF ROCK IN 2 ENTRY WHEN THE SCOOP RODE UP ON A PIECE OF ROCK CAUSING EMP TO STRIKE HIS HEA MINE ROOF. TOTAL HEIGHT OF TRAVELWAY WAS APPROX 38 IN AT THIS POINT
