EE WAS CHANGING A TIRE ON A CAT HAULER, HE HAD A 3/8 CHAIN LYING ON TOP OF THE TIRE. HE WAS USING A LOADER TO ASSIST HIM. THE CHAIN FELL OFF AND CAUGHT HIS HAND BETWEEN THE CHAIN AND THE BACK OF THE LOADER BUCKET.
EMPLOYEE WAS LOADING A 777 CAT HAULER W/A 988B LOADER. HE HAD A LARGE ROCK IN THE LOADER BUCKETAND HE DROPPED THE LOADER BUCKET TOO FAST AND HIT THE HAULER BED. THIS FALL CAUSED A JARRING AFFECT THAT INJURED HIS NECK.
EE HAD PULLED HIS TRUCK ONTO THE DUMP AREA & STARTED BACKING UP TO THE DUMPING SITE. AS HE WAS BACKING BACK THE ROAD SPLIT & HE BACKED BETWEEN THE SPLIT OF THE ROADS THE TRUCK TURNED OVER ONT O ITS SIDE
