Employee was traveling underground on personnel carrier to begin shift. He stopped to allow another employee to exit carrier and allowed carrier to roll backwards. He caught himself between carrier and mine top.
Employee stated that he bumped his head against mine top while operating scoop.
EMPLOYEE WAS CLEANING RIB AREA WITH SCOOP AND SOME DEBRIS HIT HIM IN FACIAL AREA.
EMPLOYEE WAS RIDING MANTRIP INSIDE TO BEGIN WORK DAY. HE STATED THAT MANTRIP WENT THROUGH WATER HOLE AND THAT HE RAISED UP TO KEEP FROM GETTING WET, WHICH CAUSED HIM TO CATCH HIS BODY BETWEEN MINE ROOF AND MANTRIP.
EMPLOYEE WAS OPERATING SCOOP AND TRAVELING DOWN GRADE. HE STATED THAT HE BEGAN TO SLIDE INSIDE OF SCOOP AND REACHED OUT TO HOLD ONTO SCOOP AND CAUGHT THE TIPS OF HIS FINGERS.
EE WAS ENTERING MINE & RAN OVER A PIECE OF DRAW ROCK CAUSING HIM TO BUMP HIS HEAD RESULTING IN SORE NECK MUSCLES.
EMPLOYEE STATED THAT HE HAD BEEN BOLTING ROOF TOP IN #6 HEADING. HE HAD WENT TO LOAD GIVE ON BUGGY AND BUMPED HIS HEAD AGAINST LOW SPOT IN ROOF TOP.
