Employee was attempting to cross the low belt behind the dolly. The haulage unit moved while in the process and pinched the man's foot between the dolly and the low belt structure.
While riding in a battery track mantrip, the EE tried to reposition himself when the mantrip stopped. While in this process, the mantrip started to move again and he jarred his head against the roof.
Employee was operating a scoop bringing supplies to the section when he ran over a crib block causing him to jar his back.
EE was operating a scoop recovering belt materials, scoop became entangled with the life line and EE did not see that it was caught. He moved forward until the rope streched so tight it broke sending a cone flying and striking him on the left shoulder. EE started losing time on 8/4/08.
While pulling mt cars out of the b main area, the cars roofed causing the man to strike his right hip against the locomotive. Employee missed no work until problem arose again and he started missing work on 10/13/08.
Traveling on a Long Airdox 3-wheeled jeep and hit a rut in the roadway causing the employee to be bounced in the air and landed hard on his tailbone.
While delivering a 100' piece of belt in a mantrip/locomotive the drag came loose(It was the wrong direction, dropped into the bottom, raised the locomotive to the roof catching the belt and pulling it over the operator's compartment.
Employee was turning the bull gears on a belt drive while other man were threading a pull rope to thread the belt thru the drive rollers. His hand slipped causing his right index finger to get caught between the teeth of the bull gears.
While operating a marmon 3 wheeler the front wheel hit a hole and made the steering wheel spin out of control, striking the employee s thumb.
While tightening the belt up on the continuous haulage unit and attempting to add a set of hand-rails, the chain slipped causing the structure to move quickly towards the drive catching the employee's ring finger of the left hand.
THE EMPLOYEE WAS OPERATING A SHUTTLE CAR. WHILE LEAVING THE LOADING POINT & TRAVELING BACK TO THE MINER, THE BOOM OF THE SHUTTLE CAR CAUGHT A POWER CABLE THAT WAS HUNG ACROSS THE ROADWAY. THE CABLE PULLED ACROSS THE OPERATOR COMPARTMENT OF THE SHUTTLE CAR STRIKING THE EE IN THE HEAD & NECK.
WHILE TRAMMING SCOOP, EMPLOYEE JAMMED HEAD AGAINST THE MINE ROOF. EE WAS EXAMINED BY A DR. & RELEASED FOR LIGHT DUTY WORK. PAIN PERSISTED SO EE WILL BE OFF WORK.
WHILE HAULING COAL THE EMPLOYEE HIT A SERIES OF HOLES IN HIS RUNWAY JARRING HIS LOWER BACK.
CREW OF MEN WERE ADDING BELT & STRUCTURE TO LOW LOW BELT. AFTER PULLING BELT UP TIGHT WITH CHAIN AND SCOOP TRACTOR EE WENT TO UNHOOK CHAIN. SCOOP DRIFTED BACK ABOUT THREE INCHES AND PINCHED FI NGER BETWEEN TAIL AND CHAIN. INVESTIGATION FOUND NOTHING WRONG WITH BRAKE SYSTEM ON EQUIPMENT.
EMPLOYEE WAS TRAINING SLOPE BELT USING A HAMMER WITH TAPE AND GROVES CUT IN HANDLE. EMPLOYEE WAS HITTING BOTTOM HANGERS AND HAMMER GOT CAUGHT BETWEEN BELT AND ROLLER, PULLING HAND INTO BELT. E MPLOYEE WAS WEARING A WATCH WHICH CAUSED INJURIES WHEN HE PULLED HAND AND WRIST OUT.
2 MECHANICS WERE CLEANING FIRE SUPPRESSION SPRAYS ON SIDE OF MINER. THEY HAD THEIR MANTRIP PARKED BESIDE MINER AT AN INTERSECTION. ALONG SIDE MANTRIP WAS A CANVAS CHECK-FOREMAN WAS OPERATING S COOP, REMOVING GOB FROM FACE OF #1 ENTRY AND HE DROVE SCOOP THROUGH CHECK LIFTING MANTRIP WITH SCOOP PUSHING MANTRIP INTO LEG OF MECHANIC.
