EMPLOYEE PLACED A SHEET OF METAL ON A WOOD BLOCK TO MAKE A RAMP FOR A 330M 100 TON ROCK TRUCK TO PULL ON. AS THE TRUCK PULLED ON THE WOOD BLOCK SLIPPED OUT STRIKING HIS RIGHT UPPER LEG CAUSING TISSUE DAMAGE.

EE STATED HE ATTEMPTED TO MOVE A LARGE RCK WITH LOADER BUCKET WHEN THE BUCKET SLIPPED CAUSING THE LOADER TO MOVE UPWARD QUICKLY CAUSING HIM TO FEEL PAIN IN HIS NECK.

EE STATED HE PERFORMED VARIOUS SERVICE JOBS 8/20/02, WORKED HIS FULL REGULAR SHIFT, WENT HOME & AFTER HE SHOWERED HE NOTICED HIS EYE WAS RED. HENOTICED A PARTICAL OF FOREIGN MATERIAL IN HIS EY E. HE FLUSHED IT, REPORTED FOR WORK 8/21/02 AND HAD EMT FLUSH IT-- THEN WENT TO DR. AND THE OBJECT WAS REMOVED. HE WAS RELEASED TO WORK 8/23/02.
