IE was traveling down the #2 entry when IE contacted a short pan sticking out of the rib with IE's right arm just above the elbow on the inside that required 13 stitches to close the wound.

IE was tramming a skid loader when the cab contacted a multi-hook raising the cab of the skid loader and hinging it back. When the cab came free from the multi-hook it slammed down smashing the IE's left foot.

While traveling into the section the door of the man trip portion or dog box came open both employee's in that compartment grabbed the door to close it, the one employee did not get his finger out of the way in time slamming in the door as it closed. No fracture but it did cause a laceration that required sutures to close the wound.

The employee was operating a scoop underground and was articulating the machine when he placed his hand between the door and the frame. The tire of the machine hit the door and pinched his finger between the two. This caused a laceration to his right little finger which required sutures. The employee returned to his regular job. MSHA reportable.

The employee was traveling back underground with a load of material when the brakes failed on the truck. The truck ran into the left rib causing the steering wheel to spin around and strike his right hand. MSHA restricted duty 7-19-08.

The employee was tramming the shuttle car in the section when she hit a pot hole. She bounced up and hit her head on the cab causing pain in her neck and shoulders. She was placed on restricted duty 01-10-06.

EE WAS OPENING A DOOR UNDERGROUND SO A TRACTOR COULD GO THROUGH. THE TRACTOR OPERATOR STRUCK THE DOOR AND THE EE'S HAND WAS SMASHED BETWEEN THE DOOR AND THE RIB. THE EE WENT TO THE DOCTOR, X-RAYS WERE NEGATIVE AND NO STITCHES WERE REQUIRED. EE WAS PLACED ON RESTRICTED DUTY 5/13/05.

THE EE WAS OPERATING THE LAND ROVER ON A GRADE UNDERGROUND. THE PIECE OF EQUIPMENT STARTED PICKING UP SPEED. THE EE UNABLE TO SLOW IT DOWN, DECIDED TO EASE THE EQUIPMENT INTO THE RIB TO STOP IT. THE FORCE OF THE MACHINE HITTING THE COAL RIB CAUSED THE ENGINE END OF THE EQUIPMENT TO TURNOVER, WHICH IN TURN CAUSED HIS LEFT HAND TO HIT THE FRAME OF THE CAB RESULT IN SOFT TISSUE INJ.

THE EE WAS GETTING A BAG OF ROCK DUST OUT OF THE BUCKET OF THE WAGNER. HE PLACED HIS FOOT UNDER THE BUCKET AS HE WAS LIFTING THE ROCK DUST BAG OUT. THE OPERATOR OF THE WAGNER LOWERED THE BUCKE T AT THE SAME TIME & CAUGHT THE EE'S FOOT BETWEEN THE BUCKET & FLOOR. X-RAYS WERE TAKEN. THE FOOT WAS NOT BROKEN. HE WAS PLACED ON RESTRICTED DUTY. 1ST DAY RESTRICTED DUTY 8/14/02.

A BOLT ON THE SPRIN G BOARD SECTION OF THE TAIL CAME IN CONTACT WITH THE EE RIGHT FOOT WHICH RESULTED INA COMPOUND FRACTURE AND LACERATION TO THE 4TH TOE. FIRST DAY OUT 3-21-01.

THE EE WAS LOADING A BELT WINDER INTO THE BUCKETOF A WAGNER. THE WINCH CABLE GOT HUNG UP ON THE BUCKET. THE WAGNER OPERATOR SLACKED OFF THE CABLE AND THE EE TRIED TO RESET THE HOOK. HE GRABBED THE HOOK WHICH STILL HAD PRESSURE ON IT. THE HOOK CAME LOOSE SMASHING HIS FINGER AGAINST THE WAGNER BUCKE. THE EE HAD AN ABRASION TO HIS LEFT RING FINGER, AND WAS PLACED ON RESTRICTED DUTY

THE EE WAS HELPING MOVE FANS IN THE 3RD WEST SECTION. EE AND ANOTHER EE WERE IN THE PROCESS OF MOVING THE FAN ON THE LOW SIDE WHEN THE OTHER EE WHO WAS OPERATING THE WAGNER SCOOP THOUGHT HE WA S SIGNALED BY EE TO MOVE THE SCOOP FORWARD. WHENTHE SCOOP MOVED FORWARD, THE BVCKET SLID INTO OR OVER THE TOP OF EE S FOOT, WHICH RESULTED IN A SEVERE LACERATION. FIRST DAY OFF 9-26-97.

EE WAS OPERATING A SHUTTLE CAR ON ROUGH ROADWAYS. HE HIT A BUMP WHICH CAUSED HIM TO BOUNCE UP AND STRIKE THE CAB OF THE VEHICLE WITH HIS HEAD, JAMMING HIS NECK. ON 4-28-97 EE WAS DRIVING THE S HUTTLE CAR AND THE POWER KEPT KICKING. THIS AGGRAVATED HIS NECK FROM THE PREVIOUS INCIDENT. HE COMPLAINED OF A SORE NECK AND SAW A DR. ON 4-29-97.
