Employee was operating a low-trac when it quit tramming EE was being pulled outside by a diesel scoop when EE hit something jarring EE hurting lower back, butt, left leg.
Chain being used to pull shuttle car snapped and struck employee in the lower right leg.

Ee was riding the mantrip out at the end of the shift. His foot got caught on a piece of belt and pulled him so hard that he fell from the mantrip and broke his ankle. Surgery will be required and he will be off work for several weeks.
A brattice fell and hit the miner. It actually hurt his back, knee and thumb. Everything but the thumb got ok. The thumb had to have minor surgery. He missed two days for surgery and recoup and returned with no problems.
Miner was shoveling at the tailpiece when the spillboard fell and hit him on the head and shoulder. All tests (x-rays, etc.) were clear. Ended with bruises and being very stiff. Family doctor gave him a week off.

EE was operating a haul back truck. It hit a rough spot in the road which jerked him sideways in the cab. This caused a strained back. Two days off and physical therapy has allowed EE to return to work with no problems.
EE drives an off road dump truck. When he was dumping his load, the jack failed and the bed came down hard causing him to bounced in his seat. Took one day off and then came back to work, no problems.
EE drives a rock truck at a surface mine. He has expericence back strain over the years. On this date, it became acute and he has sought medical relief.
A repairman and the Ee had a scoop jacked-up on blocks. The blocks kicked out causing the scoop to drop 3 to 4 feet. Ee was in the drivers compartment. This sudden drop tossed Ee around in the compartment which hurt his back. He has a compression fracture.
While Ee was in his shuttle car, a rock came off rig and hit his leg. The cut required stiches. He returned to work later the same day.
While driving a shuttle car it hit a hole in roadway, causing EE to be thrown up into the canopy, hurting his neck and shoulder.
Ee is a truck driver. As he was waiting to get loaded a large rock fell into bed and jarred him while he sat in the cab.

A large rock came from out of the mountains (not from the permitted area) and hit the truck Ee was driving. This knocked Ee from truck causing only bursies and strains. No bones broken. No cuts, only scratches.
Employee was driving a water truck. He hit a rock in the road that caused him to be jolted so bad that it hurt his back.
A RD-800 MACK TRUCK LOADED AT THE W-1 MINE PIT & WAS GOING OFF THE HAUL ROAD WHEN DRIVESHAFT CAME OUT & THE DRIVER TRIED TO RIDE IT OUT. THE DRIVER JUMPED FROM THE TRUCK WHICH LEAD TO HIM HAVING A SERIOUS HEAD INJURY.
Worker was in a rail runner when he got his head into the top causing minor head injuries.
EE was on his way to pick up a load of rock in his truck, ee lost control of truck and ran into the ditch line injuring his arm and leg in cab of truck. This accident was reported to us on 10-18-04.
EMPLOYEE WAS WORKING DOZER IN ROCK PILE. A DUMP TRUCK WAS UNLOADING WHEN HIS BRAKES FAILED AND HIT THE DOZER KNOCKING EMPLOYEE AGAINST THE SEAT, SCRAPING AND BRUISING HIS LEFT THIGH. THIS ACCIDENT WAS REPORTED TO OUR COMPANY ON 10/18/04.
Employee was greasing the tailpiece of the belt-line when his sleeve got caught in the moving belt line and jerked his arm and pulled his shoulder.
EE WAS DRIVING A SHUTTLE CAR, HIT A HOLE AND IT JAMMED HIS NECK. WENT TO LOCAL DOCTOR TO GET CHECK OUT. NO LINGERING PROBLEMS WITH NECK.
WHILE DRAGGING CABLE WITH CAR, A PIECE FLEW UP & HIT EE IN MOUTH & BROKE HIS TEETH OUT. HE WENT TO A DENTIST FOR DENTURE REPAIRS. NO OTHER PROBLEMS.
EE GOT HIS BRIDGE TOO CLOSE TO THE LEFT RIB AND GOT PINNED BETWEEN RIB AND CONTROLS ON BRIDGE. HE WAS ABLE TO HIT PANIC BAR QUICKLY ENOUGH TO AVOID SERIOUS INJURY. HE CAME BACK TO WORK ON 5-29 -02.
