HE WAS HOLDING THE CRIMPED END OF THE HOSE WHEN THE CRIMPED END BLEW OFF CAUSING THE GREASE GUN AND HOSE TO STRIKE HIM ON THE RIGHT FRONT SHOULDER. WHEN IT HIT THE GROUND, THE GUN AND HOSE WERE SEPARATED.
SHE WAS TURNING AROUND IN WATER TRUCK AT END OF HAUL ROAD BEOND PIT AREA. DUE TO FOG SHE COULDN'T SEE & RAN OFF SIDE OF ROAD CASUING WATER TRUCK TO ROLL.

CLAIMS THIS INCIDENT OCCURRED DURING A PERIOD OF TWO TO THREE MONTHS CAUSED BY DRIVING OFF HIGHWAY EQUIPMENT.
THE EMPLOYEE NEEDED TO CLEAN OUT THE FEEDER. HE SIGNALED THE LOADER OPERATOR NOT TO FEED THE HOPPER. AFTER 5 MINUTES THE LOADER OPERATOR SAW THE FEEDER RUNNING & ASSUMED IT WAS ALRIGHT TO DUMP . THE EMPLOYEE WAS IN THE FEEDER AND SOME OF THE MATERIAL DUMPED STRUCK THE EMPLOYEE
EE WAS DISLODGING A ROCK FROM THE SIDE OF A HEAD PULLEY. THE BELT WAS STOPPED. THE BELT MOVED SLIGHTLY BACKWARD PINCHING HIS FINGER BETWEEN THE ROCK & A SIDE PIECE OF ANGLE.
