Contractor driver dumping and had stated she was uneasy in area of dumping then while lowering bed of trailer, the truck turned over resulting in damage to truck/trailer and driver experienced pain to shoulder and neck resulting in work restrictions.
EMP WALKED INTO PATH OF MOVING FORKLIFT AND WAS KNOCKED DOWN RESULTING IN ARM BEING INJURED BY WHAT X-RAYS SHOWED AS A CRUSHING TYPE INJURY (SEVERE CONTUSTIONS/BRUISING).
Driver was lifting and releasing tailgate of dump trailer in an attempt to clear dirt and debris so tailgate would close. On the last release of the tailgate the driver's left hand ring finger was caught between the tailgate and the bed of the truck causing a laceration which required stiches.
Employee while driving company van, dropped cell phone, leaned over to pick up phone and van struck berm on side of road. Employee's head hit steering wheel, causing laceration to nose, requiring stitches.
DRIVER OF DUMP TRUCK SAID HE HIT A DIP IN THE PIT, BOTTOMED SEAT OUT JARRING HIM AND HURTING HIS BACK.
EMP HAD JUST LOADED A METRIC TON BAG OF CLAY ONTO TRUCK AND WAS TRAVELING ON FORKLIFT TO GET ANOTHER BAG WHEN ANOTHER EE STEPPED FROM BEHIND A ROW OF BAGS STARTLING DRIVER CAUSING HIM TO APPL Y BRAKES TO FORKLIFT WHICH SLID ON WET CONCRETE FLOOR CAUSING THE FORKLIFT TO SLIDE JAMMING EE'SRIGHT FOOT WHICH HE WAS DEPRESSING BRAKE WITH. SPRAINED RIGHT ANKLE AND LEG PAIN.
AFTER MAKING TURN, STEERING WHEEL CAUGHT RIGHT WRIST AT THUMB ON MIDDLE SECTION OF STEERING WHEEL. AFTER SEVERAL DAYS DECIDED TO SEEK MEDICAL ATTENTION DUE TO DIFFICULTY IN PICKING UP OBJECTS AND PAIN EXPERIENCED WHEN USES HAND IN ANY WAY.
EE WAS TRAVELING FROM MAIN ROAD INTO PLANT ENTRANCE AROUND RECEIVING WHEN A COMPANY TRUCK DRIVEN BY A CONTRACTOR EE WAS TRAVELING IN A WESTWARD DIRECTION TOWARD HER. HE DID NOT SEE HER AND STR UCK HER IN THE BACK PASSENGER SIDE OF HER VEHICLE. SHE COMPLAINED OF BACK PAIN.
