A 24" diameter by 9' long screw section was being lifted by a picker. When lifted the screw swung unexpectedly striking a mechanic on the left side, pushing his right side into a handrail. This resulted in a broken rib on the left side.

Struck ribs against the operators compartment of the shuttle car, when shuttle car lost power, broke 2 ribs, and bruised spleen.

Employee had backed out from a parking space near the tool room, turned and was moving forward to exit the warehouse parking lot when the truck struck the concrete retaining wall that runs east west to allow lower elevation access to the receiving dock.

While the employee was traveling to the mining section to begin work the nut retaining the pitman arm fell off causing the jeep to veer to the left climbing the rib and rolling over. Initial examination at the hospital showed no injury. Reexamination on 7/8/11 showed a fractured vertebrae. At that time the doctor put him off work.

Employee was driving down the drift when a piece of warning flagging got under his glasses, abrading his eye. Employee got first aid treatment that day and then was given Rx eye drops a few days later from a 2nd HCP. Differing medical opinions make it unclear if this injury treatment was a result of the abrasion and whether or not it meets MSHA's recording criteria.

The Versi-Track was being lowered down 9 shaft. Ran out of cable so operator called & asked if they could see it. Employees moved to shaft perimeter to look; it hadn't reached bottom. Operator started to move back up; some material dribbled down, the people moved & a big piece fell, exploding when it hit bottom striking all people & causing a laceration to one person's shin.

While a utility operator was waiting to stock the bolter the shuttle car threw a loop causing the cable reel to malfunction. The cable pulled tight, broke the car shock and struck the utility operator in the face.

Employee reported pain in R Knee after kneeling. Later employee bounced his knee into LHD controls after driving over a wood block. Report is filed at this time because FMC has just received medical confirmation that the injury could have resulted from this incident.

EE was installing angle iron support for scaffolding. EE states Genie moved about 2 ft pinching his finger. ISI shut down manlift, tagged out & called H & E equipment to come & check out the machine. Upon investigation of the accident & a complete safety inspection of manlift, it appears to be the operators error.

Employee was lacing up air hoses between railcars. Employee put his hand in a known pinch point area (railcar couplers). The slack rolled in and pinched the pinky, ring and middle finger on his right hand. Causing a laceration to his right ring finger that required seven stitches.

Employee was helping crew stand a roll of conveyor belt upright. Chain was wrapped around the roll & the LHD started to back up. The roll slipped breaking the chain. Chain whipped around striking employee in the left ear & forehead.

A Sein Brute 612 mantrip carrying two miners was traveling at 38-39 xc, 592 S.Longwall Panel 3 Road (1 Room). A mechanical failure within the steering mechanism resulted in the over-turning of the mantrip. A miner was pinned under the mantrip resulting in fatal injuries. The other was ejected, sustaining injuries. The company is investigating this incident with MSHA.

While sitting in back of truck being hauled into plant, the driver hit a pot hole, bouncing the people in back of truck. He hit his back right side middle on the plywood bench.

Unloading drag conveyor when it got hung up. Employee climbed about 4 ft. up ladder to look in conveyor. Other employees bumped conveyor. Upon energizing conveyor the stabilizer to the gear box collapsed allowing box to spin 35-40 degrees. The gear box struck employee knocking him off the ladder. When others found him he was laying flat on back; ladder was still in position.

Employees were traveling in vehicle when it lost power. They stopped vehicle, set parking brake, chocked wheels & then looked under hood to identify problem. Found cable wasn't connected to battery. As they were attaching cable to the battery vehicle was struck from behind by another vehicle. Injured employee received fracture to L foot & laceration to R elbow.

Employee was attaching a cookie sheet to an Eimco 936 and a chain harness that was stored on the front of the 936 became tensioned. The chain harness slipped off the post it was hung on striking the person in the chest, knocking him into the rib. He suffered a: bruised heart, broken ribs and a slight tear to the liver.

Changing a bent push ram bolt on #126 support. Relay bar needed to be retracted to release the pressure from the bolt. This was deacitvated but the push ram pushed out because of a bad spool valve putting immediate pressure on the bolt. EE put left hand on end of bolt to pull it out. Bolt was pushed into the face conveyor, pinching his finger between bolt & face conveyor.

EE WAS TRAMMING THE BOBCAT WHEN A FAULTY SAFETY SWITCH CAUSED THE HYDRAULICS TO LOCK UP WHICH SET THE BRAKES, CAUSING THE BOBCAT TO STOP IMMEDIATELY AND UNEXPECTEDLY. THE SUDDEN STOP CAUSED TH E OPERATOR TO HIS HIS HEAD INTO THE TOP OF THE CAB. NOTE: WE WERE NOT INFORMED THAT THIS EE WAS ON RESTRICTED DUTY UNTIL 2/2/4 EVEN THOUGHT THE RESTRICTIONS BEGAN ON 1/20/4.
