An employee operating a 992 Cat Loader was trying to put a vent cover that had falling off of the AC unit during operations. As EE was trying to snap the cover back into position the cover slipped and the momentum the employee was using caused EE's hand to go inside the vent resulting in EE's hand coming in contact with the fan cutting EE's index and middle fingers.
