Loader Operator backed down the feed ramp for the mill with a loader as a contractor dump truck was backing into the bay perpendicular to that ramp. The dump truck struck the side of the loader tire and counterweight shifting it to the right and in the process strained the muscles in operator's lower back.

Driver was travelling from the mine to the plant with a full load. The mine vehicle was approaching an intersection when a pickup truck crossing in front of mine vehicle failed to stop at the stop sign. The mine vehicle did not have a stop sign and did not have enough time to react to the other vehicle before impacting the rear driver side portion of the passenger vehicle.

Employee was operating haul truck. Truck was unloaded and driving down into the mine when EE felt pain in lower back. Employee stated was unsure if it was due to twisting while reaching for something or from a bump in the road.

On 5/26/2022 a OTR Driver walked into the entrance of the portal. Company truck #470 was entering the mine and bumped into the OTR Driver. The OTR driver was taken to hospital where they were treated and released. The OTR driver suffered a laceration to the back of their head.

The IE was heading back into the mine and failed to lower the bed on a Cat Articulated Haul Truck. The bed of the truck struck an I Beam support stopping the truck, causing the cab to roll on its side. The IE was wearing a seatbelt.

Employee reached onto conveyor to remove fallen empty bag. The employee's left hand became entangled into the chain conveyor and passive transfer roller resulting in laceration to palm of left hand.

A passenger vehicle was traveling south on Illinois 57 when the vehicle veered out of the lane and hit a retaining wall on mine property. The vehicle burst into flames, killing both the driver and a passenger. Neither of the individuals had any affiliation with the mine. Aside from proximity to the road, the mine had no involvement in the accident.

Employee reported on August 10, 2015 to his Supervisor that he hurt his back while driving the endloader the last week of July, 2015. Employee believes he twisted his back while pushing up product being unloaded from the barges onto the stockpile. It started out as an achy joint, but has gotten worse. Employee did not communicate this to anyone before August 10, 2015.

Performing maintenance on road grader mechanic sustained injury to his left hand. A team of two mechanics were replacing/repairing the axle on the road grader. The forklift operator was positioning the tire out of the way the tire came off the forklift on unlevel ground and kicked back pinching his hand between the tire and the equipment.

Driver took load 2 Quarry hit bump, hit head on the roof. Adjusted seat max height. Hit brakes; Shoulder hit steering wheel-pain neck&left shoulder. Stated speed~5mph. Pain in right arm also. Taken to Clinic-Xrays & medications. Lift restrictions. Steering wheel flat&no tilt. Tried to simulate. Was suspect & 8/15 WC determined head & neck would be covered not shoulder.

Dump truck was crossing a railroad track inside the plant. A heavy rain had washed out the grade crossing site. The site was covered with standing water. It was rough enough to jerk the steering wheel from the driver's hands, causing the driver to sprain her left thumb. Driver placed on light duty - no driving.

An employee operating a forklift backed into a another forklift that was parked and loaded with empty pallets. The impact caused the parked forklift to move forward and strike an employee who was walking between the parked forklift and a guardrail. The employee was caught between the pallets on the forklift and the guardrail and suffered bruises to his ribs and back.

Employee was loading bulk trailer when driver removed chocks and stop sign. Employee jumped so he would not get crushed by ramp and hit the safety rail on trailer, causing a puncture wound to his right temple area and right thigh area. Employee received 3 to 4 stitches on each wound.

HE PROCEEDED TO BACK UP WHILE THE GATE OF A PROPANE TANK HOLDER WAS OPEN (ABOUT 2 1/2 FT IN LENGTH). THE GATE STRUCK THE EE IN THE BACK CAUSING A DEEP BRUISE.

EE WAS WORKING IN THE BAGGER WAREHOUSE. HE WAS PREPARING TO LOAD BAGGED PRODUCT INTO A BOXCAR AND PROCEEDED TO MOUNT A FORKLIFT. AS HE WAS POSITIONING HIMSELF IN TEH SEAT OF THE FORTRUCK, HE F ELT A SHARP PAIN INHIS LEFT HIP AND LEG. EE RECEIVED MEDICAL TREATMENT AND WAS ORDERED TO HAVE RESTRICTED WORK ACTIVITY FOR 3 DAYS.

EE WAS PREPARING STOCK AT #5 BAGGER. HE PLACED THE PALLET ON THE STRETCH WRAPPER TO BE WRAPPED. HE NOTICED THAT FILM WAS NOT WRAPPING PROPERLY, SO HE REACHED OVER TO ADJUST FILM. HANDRAIL GAVE AND HE WAS CAUGHT BETWEEN SAFETY HANDRAIL AND CONVEYOR. LACERATION TO LEFT THIGH - 8 SUTURES.

EE PULLED GOLF CART INTO BLIND SPOT OF A 980 FRONT END LOADER AND THE LOADER STARTED IN REVERSE TO TURN AROUND. THE LOADER REAR TIRE STRUCK THE GOLF CART WHICH PUSHED THE STEERING COLUMN INTO THE EMPLOYEE'S RIGHT LEG CAUSING A FRACTURE.
