1) The employee was standing with one foot on the framework of a conveyor and the other on a ledge of a tunnel. 2) The employee was elevated off the ground. 3) The conveyor was running while performing maintenance. 4) He suffered an almost severed pinky finger, lasceration on the palm, belt abrasions and swelling on shoulder and elbow.

THE LABORER WAS SHOVELING A CONVEYOR THAT HAD NOT BEEN PROPERLY LOCKED OUT. THE BELT WAS ENERGISED AND HIS HAND WAS CAUGHT IN THE TAIL PULLEY.
EMPLOYEE WAS HELPING TO SET UP ROCK CRUSHER. A 988B LOADER BUCKET WAS POSITIONED OVER A CONVEYOR THAT WAS ALREADY SET IN PLACE WITH TAIL SECTION ON BLOCKS. WHEN LOADER STARTED TO BACK UP, THE TOOTH ON BUCKET HOOKED CONVEYOR FORCING IT OFF BLOCKS. EMPLOYEE WAS COMING OUT FROM UNDER SCREEN TRAILER- HER LEG GOT CAUGHT BETWEEN CONVEYOR AND SCREEN TRAILER.
