DOCTOR ADVISED EMPLOYEE OT AVOID HEA VY LIFTING FOR TWO WEEKS AND RETURN FOR FURTHE R EVALUATION. AFTER RE-EVALUATING EMPLOYEES CASE ON 12-7-92, EMPLOYEE WAS PLACED ON LOST TIME WORKERS COMPENSATION.

EMPL ABONDONED BOB CAT WITH ENGINE RUNNING. BOB CAT RAN INTO PIPE LINES OF 2 ALUMINUM SULPHATE TANKS WHICH CAUSED CHEMICAL SPILL. EMPL WAS EXPOSED TO CHEMICAL FUMES. EMPL RECEIVED PRESCRIPTION MEDICATION FOR ANITIOTICS.
