988B Loader was feeding chain feeder, employee was standing outside control house checking jaw. The 988 dumped a bucket of raw material on feed apron and a rock was projected out hitting employee on the lower right jaw causing laceration & fracture.

Employee was operating a CAT 250E articulating end dump. She was preparing to be loaded by a Kawasaki 80Z wheel loader. She was spotted by the loader, stopped and the loader operator loader her with 3/4" washed aggregate. She then left, dumped her load and reported that the loader jolted her in the cab and complained of neck discomfort.

OPERATOR WAS BERMING NEW PROPERTY THAT HAD PREVIOUS DIRT EXCAVATING ACTIVITY ON IT. OPERATOR WAS BLOCKING AN EXISTING RAMP. AS HE BACKED DOWN, HIS LEFT REAR TIRE DROVE OFF THE RAMP, APPROX A 2 FOOT DROP. THE WEIGHT OF THE LOADER SHIFTED TO THE LEFT REAR & THE LOADER LAID ON ITS LEFT SIDE.

9-9-00 EE CALLED IN SICK BECAUSE HIS BACK HURT. HE SAID HE COULD NOT RETURN UNTIL 9-12-00. ON HIS RETURN HIS FOREMAN ASKED IF BACK AILMENT WAS WORK RELATED. EE'S RESPONSE WAS THAT IT WAS NOT. 9-9-00, I SPOKE WITH DR. HE SAID EE HAD 2 HERNIATED DISKS. IN MEETING, EE COMPLAINED ABOUT HIS TRUCK. HE WAS GIVEN NEW TRUCK. WORKER'S COMP ACCEPTED BACK AS WORK RELATED. EE OFF UNTIL FULL REL
