Miner was removing a cap to free the journal from the anchor box. The anchor box was hanging from a cherry picker attached to the forks of a lift truck. The forks suddenly and unexpectedly dropped and cherry picker fell on the miner's hand pinching it between the journal and the cherry picker.

EE was to tighten the belts on the blower. He turned off and locked out the blower. The belt was still turning because of vacuum from the dust collector. EE grabbed the belt and it pulled his fingers into the pulley.
