FORKLIFT AND TRUCK DRIVER WERE MOVING IN THE SAME DIRECTION TOWARDS ENTRANCE RAMP TO PLANT. AS FORKLIFT STARTED UP RAMP, THE OPERATOR REMEMBERED A JOB HE NEEDED TO DO. HE PUT THE FORKLIFT IN R EVERSE AND WITHOUT SUFFICIENTLY LOOKING BACK, PROCEEDED BACK DWON THE RAMP. HE BACKED INTO THE TRUCK DRIVER AND, BEFORE HE COULD STOP, KNOCKED HIM DOWN AND RAN OVER HIS LEFT FOOT AND ANKLE WI
