Showing all 4Worker was fatally injured while driving a CAT 769C Water Truck along a standard mine haul road, returning to the water stand to re-fill the water tank. The water truck drifted left, onto the berm, travelling approximately 160' before "cartwheeling" and ejecting the worker. The worker does not appear to have been wearing the supplied seat belt.
The employee had backed under a feed chute to get a load. He dismounted the truck to activate the feed belt. He mounted the truck and moved it to spread the load evenly as it filled. He dismounted the truck to shut off the feeder and the truck rolled forward, pinning him between the truck and a cement pillar.
THE EMPLOYEE WAS OPERATING A WATER TRUCK ON THE 1525 LEVEL. HE BACKED INTO THE 3475 ACCESS TO AN OPEN STOPE AND WENT OVER THE EDGE. HE WAS EJECTED FROM THE VEHICLE DURING THE FALL AND FELL APP ROXIMATELY 75' ONTO THE ROCKS BELOW. EMERGENCY RESPONSE TEAM MEMBERS RESPONDED IMMEDIATELY.
