The Operator looked out the side door of the Forklift EE was operating and as doing such, EE struck upper front tooth on the hand held bar that assists the operators while entering the cab, and broke part of the tooth off.
Ee was replacing hoist cylinders on a CAT 777. The cylinder was hooked to a crane. Slack in the crane attachment allowed the cylinder to move, crushing the ee's right hand between the hoist cylinder and the fuel tank. The ee was diagnosed with a fracture to the right ring finger.
Ee's haul truck was being loaded. A large boulder was placed in the bed of the truck, causing the ee to be jostled in the cab. EE reported to EE's supervisor, was seen by the EMTs and refused treatment. On February 18 EE sought treatment with a chiropractor and filed a WC claim. Investigation completed and claim was accepted 03/28/19. Reporting for medical treatment.
Truck driver was jarred while being loaded and reported a back strain.
Empl. was being loaded when a rock dropped shaking the truck. Next day at 9:00 am empl. complained of neck pain. Empl. was evaluated by Physician & released to work with cautions to avoid "jarring and neck motion". Empl. chose not to return to work at this mine and has been permanently transferred to another non-mining job.
2 Labors and skid steer operator were installing French drain around portal sets. Labor was holding fabric told the operator to back up while that was happening labor started to stand up while operator was tilting bucket back and pick hand between bucket and wall.
Ee reports was driving haul truck down ramp and swerved to avoid a rock, striking the rib, causing ee to hit left rib/chest wall on the armrest of the truck. Seen at clinic, diagnosed with a chest wall contusion and put off work.
Process operator cleaned off level transmitter for RIL feed hopper. EE noticed the level in the vessel rising and exited the platform using the access ladder. EE made it to the ground and the vessel overflowed, causing hot slurry to hit them on the right side of the body, partially filling their right and left boots with hot slurry. The hot slurry burned their bilateral lower legs.
During stemming process, EE was standing behind blast hole with EE's hand on top of shovel when skid steer operator proceeded to lower bucket to dump stemming. Employee's hand was caught in between bucket and shovel crushing EE's right hand.
Ee saw doctor on 06/27/17 and reported an injury. Ee stated to doctor that ee was operating a loader in mid-May and hit a pothole and the cab slammed down; ee struck head on the roof of the loader. EE was diagnosed with post-concussive syndrome and put off work. Site immediately began investigation which is still ongoing.
EE was driving down ramp when the vehicle hit a bump causing the EE to hit head on the roof of the vehicle, bite down, and break a tooth. Tooth number 7 extracted, no other treatment, rtw on 5/5/17 with no limitations.
Ee was driving up ramp in haul truck, and another haul truck driving down, slid and made contact with passenger side of ee's truck, jarring the ee in the cab. Seen at the ER, given meds and released to full duty. On follow-up on 01/11/17, ee placed off work.
Crew was loading compressor on semi-truck trailer and employee's left hand was pinched in-between trailer bulkhead and skid mounted compressor.
While driving the Getman flatbed on the lower Rodeo ramp between the 3620 and the 1600 levels the employee drove through a water puddle near the sumps and hit a rock that was covered by the water. The Getman bounced and the employee hit knee against the lower part of the operators compartment dash board.
EE was driving a remix truck when ee hit a pothole, causing ee to bounce in the cab and hit ee head on the truck cab. EE complained of pain in the neck, head, and shoulders. Seen at the ER, MRI negative for any acute injury, ee diagnosed with a neck sprain and put off work.
Ee was riding on crew bus that hit hole in the road. Ee braced self on the seat in front of self, dislocating left little finger. Ee given a rigid splint for the finger until able to follow up with orthopedist. Returned to full duty.

Employee states while riding in a 4 person ATV, Kubota UTV, the driver attempted to turn around. The ATV slid on ice and rolled onto its side causing the employee to strike his head.
Poor road conditions on mine site
Ee hit soft spot in road, was jostled in the seat of the haul truck, and felt immediate pain in his low back. Seen at the ER, referred to back specialist who put the ee on restricted duty and prescribed physical therapy.
Ee was operating a light vehicle that had just been serviced. He began to feel ill, and notified his supervisor. He was given oxygen on site, and transported to the ER where he was diagnosed with carbon monoxide poisoning. He was given more high flow oxygen and was released to full duty.
While lining up to be loaded, the ee backed into the shovel, clipping the bottom of the shovel bucket with the dove tail on his haul truck. He strained his left shoulder on impact. He saw a doctor on 03/09 who released him to restricted duty.
Ee was sitting in haul truck being loaded. Large boulder fell onto tail-board, jolting the truck forward and the ee hyperextended his left elbow. Seen at ER and put off work, diagnosed with a soft tissue injury.
After night shift, an ee was stepping onto the hot change bus. She shook mud off one foot, and stepped up with the other and felt a pop in the knee. She was seen at the orthopedist' s that morning and diagnosed with a possible meniscus tear. Put off work.
EE was troubleshooting a ladder on a dump truck when the ladder went in down mode, smashing the ee's left wrist. X-rays taken and ee referred to orthopedist where he was placed on restricted duty. The ee is on pre-scheduled vacation until Dec. 28.
Ee was sitting in a gator and a powder truck backed into it. Ee felt pain in neck and upper back, notified supervisor but did not wish to see doctor. Pain didn't go away, saw doctor on 12/12/14. Diagnosed with a whiplash type injury and referred for chiropractic therapy. Released to full duty.
Ee walking to buggy to leave work area; powder truck backed into ee, striking his back. Ee sought treatment 11/15/14. Diagnosed w/back strain. Seen again at ER on 11/18, diagnosed w/non-occupational gallstones. Surgery did not relieve back pain, seen on 11/25, put on restricted duty for back pain due to 11/12/14 incident.
Ee was watering the 4560 upper west griffin ramp. He turned at the top of the ramp and went back down. The ee was unable to maintain control of the equipment and the truck contacted the rib several times, coming to an abrupt stop causing injuries to the operator. Ee was seen at the ER and diagnosed with fractured ribs and a laceration to the back of his head. Ee put off work.

Miner right pinky finger - hand were crushed btwn a forklift - dumpster as he was trying to dispose of material into the Dumpster and the forklift operator accidentally lowered it onto his hand.
Ee driving down wet ramp, truck slid, ee tried to slow truck by rubbing it against right rib. Truck hit bump, bounced to left rib, and ee struck left hip against cab. He was seen at the hospital, diagnosed with a contusion, and put off work pending follow-up.
Employee was a passenger on a crew bus that hit a large pothole that jarred the employee around in the seat. She was seen initially on 01/19/14, diagnosed with a strain and released to full duty. She was seen again on 02/12/14 and give a prescription for physical therapy, making this reportable for treatment.
Ee turned off ramp and the truck did not come out of the turn, striking the rib, stopping the truck. The ee hyperextended his thumb. He was evaluated by a doctor and diagnosed with a strain to the thumb, given a brace and released to restricted duty.

While driving haul truck, left side went into ditch and ee's left arm caught in steel bar on door, straining neck. Examined, diagnosed with cervical strain and put off work due to inability to operate equipment.
